术中及术前化疗干预对进展期胃癌手术患者p53、Ki-67表达及预后的影响

Influence of preoperative and postoperative chemotherapy on p53, Ki-67 expression and prognosis in patients with advanced gastric cancer

目的分析术中及术前化疗干预对进展期胃癌手术患者p53、Ki-67表达及预后的影响。方法选取2015年1月—2016年3月本院收治的50例进展期胃癌手术患者作为研究对象,随机分为对照组和研究组各25例。对照组患者给予常规进展期胃癌手术治疗,研究组患者采取术中及术前化疗干预治疗。对比两组患者p53、Ki-67表达及预后情况。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果研究组总有效率为92.0%,高于对照组的68.0%,差异有统计学意义(P<0.05)。研究组p53、Ki-67表达水平分别为(0.497±0.285)、(0.415±0.144),显著优于对照组的(0.318±0.258)、(0.591±0.166),差异均有统计学意义(均P<0.05)。研究组患者手术预后复苏时间、焦虑评分分别为(1.0±0.2)h、(8.9±0.2)分,均优于对照组的(1.9±0.5)h、(12.1±1.3)分,差异均有统计学意义(均P<0.05)。结论在临床进展期胃癌手术患者中应用术中及术前化疗干预,不仅可降低患者p53、Ki-67表达水平,还可改善患者临床预后,发挥积极影响。

Objective To analyze the effects of preoperative and postoperative chemotherapy on the expression of p53 and Ki-67 in patients with advanced gastric cancer. Methods 50 patients with advanced gastric cancer from January 2015 to March 2016 were randomly divided into eontrol group and study group（n=25）. Patients in the control group were treated with conventional advanced gastric cancer surgery. The patients in the study group were treated with preoperative chemotherapy and chemotherapy. The expression and prognosis of Ki-67 and p53 were compared between the two groups. Measurement data using t test, count data using chi square test, P〈0.05 was considered statistically significant. Results The total effective rate of the study group was 92.0%, higher than that of the control group （68.0%）, the difference was statistically significant （P〈0.05）. The expression levels of p53 and Ki-67 in the study group were （0.497＋0.285）, （0.415±0.144）, which were significantly better than those of the control group [（0.318±0.258）, （0.591±0.166）], the differences were statistically significant （all P〈0.05）. The recovery time and anxiety score of the patients in the study group were （1.0 ± 0.2）h, （0.2± 8.9）points, which were better than those of the control group[（0.5± 1.3）h ,（1.9± 12.1）points], the differences were statistically significant （all P〈0.05）. Conclusion Preoperative chemotherapy in patients with advanced gastric cancer surgery can not only decrease the expression of p53 and Ki-67, but also can improve the prognosis of patients, and play a positive effect.
