重庆市公务员功能性消化不良患者健康相关生命质量研究

Study on the health related quality of life in civil servants with functional dyspepsia in Chongqing

目的分析重庆市公务员功能性消化不良(functional dyspepsia,FD)患者抑郁状况和健康相关生命质量(health related quality of life,HRQOL)结果 ,并研究患者健康相关生命质量的影响因素。方法选取同期参加体检的公务员831名、个体经营者925名,以公务员FD患者(208例)作为试验组,以个体经营FD患者(204例)作为对照组,使用抑郁自评量表(selfrating depression scale,SDS)评估抑郁状况,使用生活质量评价量表(short form 36 questionnaire,SF-36)测量HRQOL分数,并使用单因素方差分析和多元逐步线性回归统计方法对公务员FD患者HRQOL的影响因素进行分析。结果公务员FD患者HRQOL总分低于个体经营FD患者(t=2.95,P=0.00),SDS评分高于个体经营FD患者(t=3.03,P=0.00)。统计结果显示,SDS评分、辛辣饮食、年龄、性别、治疗处理、文化程度、行政级别、锻炼、吸烟、饮酒可影响公务员FD患者HRQOL评分。结论重庆市公务员FD患病率较高,心理健康水平和生命质量均低于个体经营者。重视中老年、高学历、高行政级别者和女性的防治、戒除辛辣饮食、戒烟、戒酒、对症药物治疗、加强锻炼、心理疏导可以改善患者健康相关生命质量。

Objective To analysis the depressive state and health related quality of life（HRQOL） in civil servants with functional dyspepsia（FD） in Chongqing,and study the influencing factors on health related quality of life. Methods The civil servants with FD were divided into experiment group,and the individual operator with FD were divided into control group. Use selfrating depression scale（SDS） to assess degree of depression,and use SF-36 scalec to assess the HRQOL. The one-way ANOVA and stepwise regression analysis were used to study the influence factors of HRQOL in civil servants with FD. Results The HRQOL scores of civil servants with FD were lower than that of individual operator（t = 2.95,P = 0.00）,and the SDS scores of civil servants with FD were higher than that of individual operator（t = 3.03,P = 0.00）. Stepwise regression analysis showed that：SDS score,spicy diet,age,sex,treatment,education,administrative level,physical exercise,smoking and alcohol consumption influenced the HRQOL scores of patients with FD. Conclusion The prevalence rate of FD of civil servants in Chongqing was high. The level of mental health and the quality of life all were lower than those of individual operator. The results suggested that more attention must be paid to the elderly,higher education,high-level positions and women. Quiting spicy food,quiting smoking and drinking,drugs treatment,psychological counseling and physical exercise all can improve patients＇ health related quality of life.
