2型糖尿病肾病的载脂蛋白E基因多态性研究

Study on apolipoprotein E polymorphism of type 2 diabetes mellitus with nephropathy

目的 研究载脂蛋白E(ApoE)基因多态性与 2型糖尿病肾病的关系。方法 采用聚合酶反应 (PCR)和限制性片段、长度多态性分析 (RFLP)技术 ,研究 5 8例 2型糖尿病肾病患者、5 6例 2型糖尿病未合并肾病患者和 76例正常对照者载脂蛋白 (ApoE)的基因多态性。 结果 ① 2型糖尿病肾病患者ε2 等位基因频率 (14 6 % )高于 2型糖尿病非肾病患者 (3 6 % ) (P <0 0 1,χ2 =8 37)和正常对照者 (5 7% ) (P<0 0 5 ,χ2 =4 2 8) ,3组中ε3 和ε4等位基因频率差异无显著性 (P >0 0 5 )。②携带ε2 等位基因的 2型糖尿病患者TG水平较携带ε3 和ε4等位基因的 2型DM患者增高 (P <0 0 1vsP <0 0 5 ) ,而LDL C水平降低 (P <0 0 5 ) ,携带ε4等位基因的 2型DM患者 ,其TC和LDL C水平较携带ε2 和ε3 等位基因的个体增高 (P <0 0 1)。③ApoE基因多态性在男女性别上差异无显著性 (P >0 0 5 )。结论 ApoE基因多态性与 2型糖尿病肾病的发生有关 ,提示ε2 等位基因频率越高 ,发生

Objective To investigate the relationship b et ween the genetic polymorphism of apolipoprotein E and type 2 diabetic nephropath y. Methods The genetic polymorphism of apolipoprotein E in 58 t ype 2 diabetic patients with nephropathy, 56 type 2 diabetic patients without ne phropathy and 76 normal controls were observed by the method of polymerase chain reaction restriction fragment length polymorphism.Results ① The ε 2 allele frequency of type 2 diabetic patients with nephropathy (14 6 % ) was significantly higher than that of type 2 diabetic patients without nephrop athy (3 6%) ( P <0 01 χ 2=8 37) and normal controls(5 7%) ( P <0 05 χ 2=4 28). There were no significant differences of ε 3 and ε 4 allele fr equencies between the groups.( P >0 05). ② The level of plasma TG of type 2 diabetic patients with ε 2 allele was significantly higher than that of ty pe 2 diabetic patients with ε 3 allele or ε 4 allele ( P<0 01 vs P <0 0 5 ), while the level of plasma LDL C was significantly lower than that of type 2 d iabetic patients with ε 3 or ε 4 allele ( P <0 05). The levels of plasma TC and LDL C of type 2 diabetic patients with ε 4 allele were significantly higher than those of type 2 diabetic patients with ε 2 or ε 3 allele( P <0 01). ③ There was no significant difference of ApoE genetic polymorphism bet ween male and female ( P >0 05) Conclusion Apolipoprotein E polymorphism is related to type 2 diabetic nephropathy.ε 2 allele is a risk factor for type 2 diabetic nephropathy.
