补中益气汤联合西药治疗功能失调性子宫出血随机平行对照研究

Combined with Western Medicine Buzhong Yiqi Tang(补中益气汤) Dysfunctional Uterine Bleeding Randomized Controlled Study

[目的]观察补中益气汤联合西药治疗功能失调性子宫出血疗效。[方法]使用随机平行对照方法,将84例门诊患者按就诊顺序号方法简单随机分为两组。对照组42例黄体酮,5~10mg,1次/d,注射4~6d;安宫黄体酮片,8~10mg/次,每6~8h服药1次,连续服用3~4次止血后逐渐减量,每3d减1次,每次按1/3递减至维持剂量4~6mg,维持10~12d;停药后发生撤退性出血（止血效果在撤退出血停止后出现）;丙酸睾酮,25~50mg/次,肌注,隔日1次,共3d,撤退出血时不应再用雌、孕激素。治疗组42例补中益气汤（黄芪、人参/党参、炙甘草各15g,白术、当归各10g,陈皮、升麻各6g,柴胡12g,生姜9片,大枣6枚;头痛加川芎、藁本、蔓荆子、细辛以散风止痛;腹中痛加白芍以柔肝止痛;咳嗽加五味子、麦冬以敛肺止咳;气滞加木香、枳壳以理气解郁）,1剂/d,水煎100m L,早晚空腹时温服;西药治疗同对照组。连续治疗1个月经周期为1疗程。观测临床症状、不良反应。连续治疗3疗程（3个月经周期）,判定疗效。[结果]治疗组痊愈8例,显效14例,有效16例,无效4例,总有效率90.48%;对照组痊愈4例,显效15例,有效11例,无效12例,总有效率71.43%;治疗组疗效优于对照组（P〈0.05）。不良反应对照组4例,头痛1例,头晕2例,恶心1例,不良反应发生率9.5%;治疗组5例,头痛2例,头晕1例,恶心2例,不良反应发生率11.9%。[结论]补中益气汤联合西药治疗功能失调性子宫出血,疗效满意,无严重不良反应,值得推广。

Treatment group of 42 patients Buzhong Yiqi Tang （astragalus, ginseng Codonopsis, licorice Sunburn the 15g, Atractylodes, Angelica 10g, dried tangerine peel, Cimicifuga each 6g, Chai Hu 12g, ginger 9, jujube 6; headache plus Chuanxiong, Ligusticum, Vitex, Asarum with scattered wind pain; belly pain added to TGP Softening pain; cough plus Schisandra, Radix to convergence lung cough; qi stagnation plus woody, Citrus aurantium to qi stagnation ）, a/d, decoction 100mL, sooner or later fasting warm clothes; western medicine with the control group. Continuous treatment of a menstrual period for a course. Observing clinical symptoms and adverse reactions. Continuous treatment of three courses （three menstrual cycles）, to determine efficacy. [Results] The clinical efficacy of the treatment group than the control group （e〈0.05）. Adverse reactions in the control group 4 cases, headache in 1 case, dizziness two cases, nausea in 1 case, adverse reaction rate of 9.5%; the treatment group, 5 cases of headache in 2 cases, dizziness, 1 case of nausea in 2 cases, adverse reaction rate of 11.9%. [Conclusion] Buzhong Yiqi Tang combined with western treatment of dysfunctional uterine bleeding, results were satisfactory, no serious adverse reactions, should be promoted.
