针刺联合中药熏洗治疗腰椎间盘突出症随机平行对照研究

A Randomized Parallel Controlled Study on Acupuncture Combined Chinese Medicine Fumigation Treatment of Lumbar Disc Disease

[目的]观察针刺联合中药熏洗治疗腰椎间盘突出症疗效。[方法]使用随机平行对照方法,将108例住院患者按病志号抽签方法简单随机分两组。对照组50例针刺（取患侧肾俞、气海俞、突出相应节段夹脊穴、次髎、秩边、环跳、委中、阳陵泉、悬钟）,腰臀部穴位进针后使针感下传,轻微刺激1~2次,平补平泻,每10min行针1次,留针30min,1次/d。治疗组58例中药薰洗（红花、艾叶、川芎、秦艽、透骨草、刘寄奴、地鳖虫、制川乌、花椒、威灵仙各20g）,煎煮30min,薰洗患处30min,1次/d;针刺治疗同对照组。连续治疗10d为1疗程。观测临床症状、腰椎疾患治疗成绩评分、症状及功能评分、不良反应。治疗1疗程,判定疗效。[结果]临床疗效治疗组优于对照组（P〈0.05）。腰椎疾患治疗成绩评分两组均有明显升高（P〈0.01）,治疗组升高优于对照组（P〈0.01）。症状及功能评分两组均有改善（P〈0.01）,腰痛、步态、下肢疼麻、日常活动影响治疗组改善优于对照组（P〈0.05,P〈0.01）,临床检查两组间无明显差异（P〉0.05）。[结论]针刺联合中药熏洗治疗腰椎间盘突出症,疗效满意,无严重不良反应,值得推广。

The control group of 50 cases of acupuncture （affected side Shenshu, Qihaishu Jiajixue Ciliao Zhibian Huantiao Weizhong Yanglingquan Xuanzhong）, Skin routine disinfection, Waist hip point into the needle after make the needle feeling down, Mild stimulus 1-2times, mild reinforcing and attenuating, Every 10 min line needle 1 time, Retaining needle 30 min, 1/d. 58 cases in treatment group treated with Chinese medicine is medicine（Safflower, argyi and rhizoma chuanxiong, radix gentianae maerophyllae, tougucao, diverse wormwood herb, ground beetle, radix aconiti, Chinese prickly ash, clematis roo,Each 20 g）.Decoction for 30 min, wash affected area 30 rain, 1/d. Acupuncture treatment with the control group.Continuous treatment of 10 d to 1 course of treatment. Observation of clinical symptoms and treatment performance score, lumbar disease symptoms and functional score, adverse reactions. Treatment group and control group in the treatment of 1 treatment course, judge curative effect. [Results] The clinical efficacy of the treatment group is better than that of control group （P 〈0.05）. Lumbar disease treatment performance score in the two groups were significantly increased （P〈0.01）, Higher in treatment group is better than that of control group （P〈0.01）. Symptoms and functional score two groups have improved （P〈0.01）. Lumbago, gait, lower limb pain, hemp, daily activities affect the treatment group improved better than that of control group （P〈0.05, P〈0.01）. Clinical examination has no obvious difference between the two groups （P〉0.05）. [Conclusion] Acupuncture combined Chinese medicine fumigation treatment of lumbar disc disease, curative effect is satisfied, no serious adverse reactions, worthy of promotion.
