肝癌患者血清中AFP、CEA、CA125、CA199检测分析

Detection of AFP、CEA 、CA125 and CA199 in serum of patients with hepatocellular carcinoma

目的通过对肝癌患者血清中甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原125(CAl25)、糖类抗原199(CAl99)的检测分析,探索其与肝癌临床诊断与治疗中的相互关系,为肝癌的早期诊疗提供价值。方法选取2012—2014年新密市郑州新华医院、河南省省直第二医院肝癌确诊病人131例,同时选取健康体检者168例作为对照;采外周血3ml,分离血清,化学发光方法检测AFP、CEA、CAl25、CAl99的水平。结果肝癌血清中AFP、CEA、CA125、CA199的水平较健康对照组升高(P<0.05),而且肝细胞癌病人组血清中AFP水平升高明显,差异有显著性(P<0.01),而肝内胆管细胞癌病人组血清中的CEA、CAl25、CAl99水平升高明显,差异有显著性(P<0.01)。结论肝癌患者血清中AFP、CEA、CA125、CA199的水平均高于健康对照(P<0.05);AFP诊断肝细胞癌价值最高,CEA、CAl25、CAl99诊断肝内胆管细胞癌的价值最高;联合检测多项肿瘤标志物,则可以提高肝脏肿瘤的早期发现率。

Objective To through detection analysis in serum of patients with liver cancer of alpha fetoprotein （ AFP）, carcinoembryonic antigen （ CEA）, cancer antigen 125 （ CA125 ）, cancer antigen 199 （ CA199）, we explore its relationship with clinical diagnosis and treatment of hepatocellular carcinoma, and the value provided in early diagnosis and treatment for liver. Methods Selected the 2012 -2014 totally 131 HCC patients from Zheng-zhou Xinhua Hospital of Xinmi, the Second Hospital to Inner Department of Henan Province, at the same time, total health examination 168 cases were selected as control. Peripheral blood 3ml and serum was separated and chemilu-minescence methods were used in the detection of AFP、CEA、CA125 and CA199 levels. Results In liver cancer serum AFP,CEA, CA125 and CA199 levels increased compared with the healthy control group （P 〈 0.05 ） , and AFP level was elevated significantly in patients serum of hepatocellular carcinoma, the difference is significant （ P 〈 O. 01 ） , and the levels of CEA, CA125 and CA199 were significantly increased in patients serum of intrahepatic eholangioearcinoma, the difference is significant （P 〈 0.01 ）. Conclusion In the serum of patients with liver cancer AFP, CEA, CA125 and CA199 levels were higher than that in healthy control （ P 〈 0.05 ） ; And AFP is the highest value in diagnosis of hepatocellular carcinoma, CEA,CA125 and CA199 are the highest value in diag- nosis of intrahepatic cholangiocareinoma; Combined detection of multiple tumor markers we can improve liver tumor early detection rate.
