卡前列素氨丁三醇注射液治疗宫缩乏力性产后出血的疗效及安全性分析

Efficacy and safety analysis of carboprost tromethamine injection in the treatment of postpartum hemorrhage caused by uterine inertia

目的探讨卡前列素氨丁三醇注射液治疗宫缩乏力性产后出血的疗效及安全性。方法选取2015年1月~2016年5月在我院分娩的100例宫缩乏力性产后出血产妇作为研究对象,按照入院顺序将其分为观察组和对照组,各50例。对照组分娩后静脉滴注缩宫素20 U＋宫肌注射缩宫素20 U,观察组分娩后静脉滴注缩宫素20 U＋宫肌注射卡前列素氨丁三醇250μg。比较两组的治疗效果、产后出血情况和不良反应发生率。结果观察组的不良反应发生率为6.0%,显著低于对照组的32.0%,差异有统计学意义（P〈0.05）。观察组的总有效率为98.0%,显著高于对照组的66.0%,差异有统计学意义（P〈0.05）。观察组产后2、24 h的出血量显著低于对照组,差异有统计学意义（P〈0.05）。结论宫缩乏力性产后出血患者采用卡前列素氨丁三醇进行治疗,可有效减少患者的术后不良反应发生率,提高治疗效果,改善术后出血状况,值得临床推广应用。

Objective To explore the efficacy and safety of carboprost tromethamine injection in the treatment of postpartum hemorrhage caused by uterine inertia.Methods 100 cases of postpartum hemorrhage caused by uterine inertia in our hospital from January 2015 to May 2016 were selected and divided into the control group and the observed group according to the order of admission,50 cases in each group.The control group was given intravenous infusion of oxytocin20 U combined with uterine intramuscular injection of oxytocin 20 U after childbirth,the observation group was given intravenous infusion of oxytocin 20 U combined with uterine intramuscular injection of carboprost tromethamine 250 μg after childbirth.The treatment effect,postpartum hemorrhage and incidence rate of adverse reaction was compared between the two groups.Results The incidence rate of adverse reaction in the observation group was 6.0%,which was lower than 32.0% in the control group,with significant difference（P〈0.05）.The total effective rate of the observation group was 98.0%,which was higher than 66.0% in the control group,with significant difference（P〈0.05）.The amount of postpartum hemorrhage after delivery of 2 and 24 hours in the observation group was lower than that in the control group,with significant difference（P〈0.05）.Conclusion Postpartum hemorrhage caused by uterine inertia patients are treated with carboprost tromethamine injection,which can effectively reduce the incidence rate of postoperative adverse reaction,improve treatment effect,improve postoperative bleeding condition,it is worthy of clinical promotion and application.
