屈光参差性弱视眼黄斑频域光学相干断层成像研究 被引量:4

Spectral domain optical coherence tomography in analyzing macular retinal thickness in anisometropic amblyopia

摘要 目的应用频域光学相干断层成像(spectral domain optical coherence tomography, SD-OCT)技术分析不同类型单眼屈光参差性弱视青少年弱视眼与对侧非弱视眼黄斑区视网膜厚度差异。方法10~18岁单眼屈光参差性弱视患者42例(42眼)(弱视组),其中近视性屈光参差性弱视18例(18眼)(近视组),远视性屈光参差性弱视24例(24眼)(远视组),对侧非弱视眼为对照组。行SD-OCT快速扫描测定黄斑中心凹区(A1)、上方内圈(A2)、颞侧内圈(A3)、下方内圈(A4)、鼻侧内圈(A5)、上方外圈(A6)、颞侧外圈(A7)、下方外圈(A8)、鼻侧外圈(A9)区视网膜厚度,并进行比较。结果弱视组A2、A5、A6、A8区视网膜厚度[(315.81±15.52)、(314.24±14.58)、(281.71±16.86)、(264.29±14.85)μm]大于对照组[(305.94±9.38)、(306.22±7.87)、(267.39±13.55)、(256.78±15.62)μm],差异有统计学意义(P〈0.05);近视组A4、A6、A7、A8区视网膜厚度[(299.33±10.90)、(272.67±13.14)、(248.33±12.56)、(251.33±8.05)μm]小于远视组[(315.50±11.16)、(288.50±16.56)、(268.00±11.88)、(274.00±10.65)μm],差异有统计学意义(P〈0.05);余区域视网膜厚度组间比较差异无统计学意义(P〉0.05)。结论屈光参差性弱视患者黄斑视网膜厚度改变具有区域特异性,SD-OCT技术可协助弱视的诊断。 Objective To compare the macular retinal thickness between amblyopic eyes and non-amblyopic eyes in adolescent patients with unilateral anisometropic amblyopia by spectral domain optical coherence tomography (SD-OCT). Methods Forty-two patients aged 10 to 18 years with unilateral anisometropic amblyopia (amblyopia group) included 24 patients with hyperopic amblyopia (hyperopia group) and 18 patients with myopic amblyopia (myopia group). And the opposite eyes without amblyopia were as controls (control group). SD-OCT was adopted to measure the macular retinal thickness in central fovea area (A1), upper inner ring (A2), temporal inner ring (A3), inferior inner ring (A4), nasal inner ring (A5), upper outer ring (A6), temporal outer ring (AT), inferior outer ring (AS) and nasal outer ring (A9) of both eyes, and were compared among three groups. Results The macular thickness values of A2, A5, A6 and A8 in amblyopia group ((315.81±15.52), (314.24± 14.58), (281.71± 16.86), (264.29±14.85) μm) were significantly thicker than those in control group ((305.94±9.38), (306.22±7.87), (267.39±13.55), (256.78±15.62) μm) (P〈 0.05). The macular thickness values of A4, A6, A7 and A8 in myopia group ((299.33±10. 90), (272.67±13. 14), (248. 33±12.56), (251. 33±8.05) μm) were significantly thinner than those in hyperopia group ((315. 50± 11.16), (288.50±16.56), (268. 00±11.88), (274.00±10.65) μm) (P〈0.05). There were no significant differences in the macular retinal thickness values among the other regions (P〉0.05). Conclusion The macular retinal thickness differs regiospecifically between amblyopic eyes and non-amblyopic eyes in patients with unilateral anisometropic amblyopia. SD-OCT is meaningful to the diagnosis of amblyopia.
