摘要
目的利用图像分析技术定量研究肺腺癌中程序性死亡配体1(programmed cell deathligand 1,PD-L1)的蛋白表达及其与临床病理参数之间的相关性,并对比PD-L1免疫组化彩色图像定量分析结果与人工判读结果的一致性。方法采用免疫组化En Vision两步法检测109例肺腺癌标本的PD-L1蛋白表达。从每张切片的肿瘤区域随机抽取5个视野进行图像采集及分析,计算每个病例的染色程度比值[(棕色区域面密度÷蓝色区域面密度)×棕色平均光密度],将该值≥0.05的病例定义为PD-L1蛋白表达图像分析结果阳性。人工判读结果系参照文献通用的阳性强度与阳性细胞百分数的乘积;实验结果均使用SAS 9.1软件分析。结果 PD-L1的染色程度比值(定量)在不同性别中具有明显差异性(P<0.05);PD-L1蛋白表达的定性结果与肿瘤的组织学分型密切相关(P<0.05);PD-L1免疫组化图像分析的定性结果与人工判读结果的一致率为88.073%(K=0.7157)。结论对肺腺癌PD-L1蛋白表达程度采用图像分析定量检测所获结果与人工判读法结果一致性较好,但前者更具有客观性和准确性。
Objective To quantitatively study the expression of programmed death ligand - 1 ( PD - L1 ) in lung adenocarcinoma and analysis the correlation between PD -L1 expression and clinical pathological parameters, as well as compare results from image analysis and tochemistry EnVison method was used to detect PD - L1 protein cinoma which were collected in China - Japan Friendship Hosp manual counting. Methods Immunohis- expression in 109 cas ital. At high magnifi es of lung adenocarcation, images from 5 fields picked randomly from each IHC section were collected and analyzed. The values of stain degree were calculated using the formula : ( surface density of brown area / surface density of blue area) x mean optical density of brown area. Stain degree 〉/0.05 was defined as a positive result. Statistical analyses were done using SAS 9. 1 software. Results values of stain degree show difference for different gender (P 〈 0.05). Qualitative results of PD - L1 from image analysis are related to histological type (P 〈 0.05). The consistent percentage between image analysis and manual counting is 88.073% (K = 0. 7157). Conclusions Analysis of PD -L1 IHC images by calculating stain degree value has a good consistency with manual counting. It is more objective and accurate than the latter.
