摘要
四肢骨巨细胞瘤是一种潜在恶性病变,以往采用刮除植骨术或骨水泥填充为主要治疗手段,其中磷酸钙骨水泥被认为是一种理想的填充材料。对于肿瘤巨大,保留关节困难者采用瘤段切除人工假体置换术可使局部复发率得到有效控制。药物方面,双膦酸盐类药物和狄诺塞麦的使用使其治疗出现巨大进展。狄诺塞麦作为一种最新上市药物,对于骨巨细胞瘤的治疗效果良好,使用该药有望进一步控制骨巨细胞瘤手术前后复发,提高治愈率。对于肿瘤巨大,难以保留关节者,今后是否可采用刮除植骨+狄诺塞麦治疗的方式,保留关节,减少人工关节的使用,提高效费比,值得期待。
Giant cell tumor（GCT） of bone is a potentially malignant lesions.Curettage with bone graft or bone cement filling is the most contemporary procedure to treat GCT.Calcium phosphate bone cement is an ideal filling material.Tumor resection and artifical prosthesis replacement can effectively controlled the local recurrence rate for those huge GCT.Bisphosphonate drugs and Denosumab make tremendous progress in the treatment of GCT.Denosumab is the latest listing of a drug to treatment GCT and have a good effect.Before and after surgery of GCT,using Denosumab can be further controlled relapse,improve the cure rate.Curettage with bone graft unite Denosumab probably is a new treatment pattern for GCT.This pattern can reserve joint,reduce the use of artifical joint and improve cost-effectiveness.
