摘要
目的 :研制一种可减轻气管插管时应激损害的热塑弹性体喉镜片。方法 :选用目前临床广泛使用的Macintosh喉镜为原型,将镜片前端左侧直角设计成弧形,之后将适合医用的柔软弹性材料,即热塑性弹性体,苯乙烯(S)-氢化丁二烯(EB)-苯乙烯(S)构成的三嵌段共聚物(SEBS)贴附于喉镜凹面表面尖端1/3部分,制作由热塑性弹性体包裹成缓冲部的咽喉镜片。选20名有10 a以上临床工作经验的麻醉医师,随机分为热塑性弹性体喉镜组(A组)和直接喉镜组(B组),每组10人,在同一模型人身上模拟气道管理,通过计算机测压软件记录插管时压力,并对2组进行对比分析。结果:与直接喉镜组插管相比,热塑弹性体喉镜组插管的冲击力、平均压力峰值力均较低(P均小于0.05)。结论:热塑弹性体喉镜片可降低插管时的作用力,减轻插管时的应激损害,临床实用价值显著。
Objective To develop a thermoplastic elastomer-laryngoscope blade to further reduce the stress impairment during tracheal intubation. Methods Macintosh laryngoscope widely used clinically had the right angle on the front left side curved, and the thermoplastic elastomer Styrene-Ethylene-Butylene-Styrene(SEBS), a soft and resilient material suitable for medical application, was attached to the concave surface of the front 1/3 part of the laryngoscope to form its buffering portion made of thermoplastic elastomer. Then, 20 anesthesiologists with more than 10 a experiences were assigned into two groups: Group A applying the thermoplastic elastomer laryngoscope and Group B applying the direct laryngoscope to the same manikin. The laryngoscopic forces of both groups were recorded by computer software. Results It's found that the impulse force, average force and peak force in Group A generated by thermoplastic elastomer laryngoscope during tracheal intubation were lower than those in Group B by direct laryngoscope, with all the values of P less than 0.05. Conclusion Thermoplastic elastomer laryngoscope can reduce the forces during tracheal intubation to relive stress impairment.
