摘要
目的探讨血清糖类抗原125(CA125)及CA199联合检测在子宫内膜癌辅助诊断中的价值及临床意义。方法选择2013年2月~2015年2月我院收治的子宫内膜癌患者、子宫良性病变患者以及来我院进行健康体检且各项指标均正常者各100例作为研究对象,分别为子宫内膜癌组、良性病变组及健康对照组,检测3组血清CA125及CA199表达水平。结果子宫内膜癌组血清CA125表达量为(219.6±71.5)U/ml,阳性率69.0%,血清CA199表达量为(101.2±43.9)U/ml,阳性率45.0%,均高于其他两组(P〈0.05);良性病变组血清CA125及CA199表达量及阳性率也明显高于健康对照组(P〈0.05)。随肿瘤进展,CA125及CA199表达水平呈升高趋势(P〈0.05)。CA125与CA199联合检测诊断子宫内膜癌灵敏度为81.0%,特异度为94.0%,阳性预测值87.1%,阴性预测值90.8%,准确度89.7%,均高于单项检测(P〈0.05)。结论血清CA125与CA199检测与子宫内膜癌的诊断及分期具有一定的相关性,联合检测可提高辅助子宫内膜癌诊断效能。
Objective To explore the value and clinical significance of carbohydrate antigen 125 (CA125) and CA199 in the auxiliary diagnosis of endometrial carcinoma.Methods The patients with endometrial carcinoma and patients with benign uterus lesions admitted to our hospital, and healthy people who underwent physical examination in our hospital from February 2013 to February 2015 were selected as an endometrial carcinoma group, a benign uterus lesions group, and a healthy control group (n=100, respectively) to detect the levels of CA125 and CA199 in the three groups, Results The level and positive rate of CA125 in endometrial carcinoma group were (219.6±71.5) U/ml and 69.0%, and (101.2±43.9) U/ml and 45.0% for of CA199. These indicators were higher than those in the other two groups (P 〈 0.05). The levels and positive rate of CA125 and CA199 in benign uterus lesions group were much higher than those in the healthy control group (P 〈 0.05). The levels of CA125 and CA199 increased along with tumor progression (P 〈 0.05). The sensibility, specificity, PPV, NPV and accuracy of combined detection of CA125 and CA199 were 81.0%, 94.0%, 87.1%, 90.8% and 89.7%, which were higher than those in the detection of single item (P 〈 0.05). Conclusion The detection of CA125 and CA199 is related to the diagnosis and staging of endometrial carcinoma to certain degree. The combined detection of CA125 and CA199 can improve the diagnostic efficiency of endometrial carcinoma.
