摘要
目的 观察门冬胰岛素30联合二甲双胍治疗高原地区2型糖尿病的效果。方法 选取在我院就诊的我青海不同地区(居住海拔在2260~4000 m)2型糖尿病患者100例,随机分为对照组(n=50)和观察组(n=50)。对照组皮下注射门冬胰岛素30治疗,观察组在对照组基础上加用二甲双胍片治疗,治疗12 w后,观察比较两组血糖、血脂、体重等指标变化以及药物不良反应。结果 两组治疗后空腹血糖(FBG)、餐后2 h血糖(2h PBG)、糖化血红蛋白(Hb A1C)均低于治疗前(P〈0.05),而观察组更显著低于对照组(P〈0.05)。两组治疗后除高密度脂蛋白胆固醇(HDL-C)外,甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)均显著低于治疗前(P〈0.05);治疗后两组的体重、腰围和BMI与治疗前比较,均显著降低(P〈0.05),而两组间比较无统计学差异(P〉0.05)。两组治疗期间均未见严重不良反应。结论 门冬胰岛素30联合二甲双胍可显著降低高原地区2型糖尿病的血糖和血脂水平,改善BMI,无严重不良反应,临床对于高原地区2型糖尿病患者同样有效。
Objective To observe the effects of insulin aspart 30 combined with metformin on the treatment of patients with type 2 diabetes in plateau. Methods A total of 100 patients, who received treatment in our hospital, suffered from type 2 diabetes and came from different regions in Qinghai Province (altitude: 2260 - 4000 m), were selected and randomly divided into a control group (n=50) and an observation group (n=50). The control group was treated with insulin aspart 30 through subcutaneous injection; on this basis, the observation group was further treated with mefformin tablets. The changes in the indicators including blood glucose, blood fat and weight and adverse drug reaction in the two groups were observed and compared 12 w after the treatment. Results The FBG, 2hPBG and HbA1c in the two groups were lower than those before the treatment(P 〈 0.05); the said indicators in the observation group were much lower than those in the control group (P 〈 0.05). Except for HDL - C, TG, TC and LDL - C in the two groups were significantly lower than those before the treatment (P 〈 0.05); the weight, waistline and BMI were also much lower than those before the treatment (P 〈 0.05); there was no significant difference between the two groups (P 〉 0.05). There was no serious adverse reaction during the treatment in the two groups.Conclusion Insulin aspazt 30 combined with metformin can significantly lower the blood glucose and blood fat of patients with type 2 diabetes in plateau, improve their BMI, cause no serious adverse reaction, and is also clinically effective on patients with type 2 diabetes in plateau.
