We report a 75 years old woman with papillary thyroid microcarcinoma, and squamous cell cancer was found in her lymph node metastases. The patient was characterized by a cervical mass, which was diagnosed as papillary thyroid microcarcinoma by fine needle aspiration. Total thyroidectomy＋right radical neck dissection＋ VI bilateral neck lymph node dissection were performed for the patient.
