肝移植受者的失眠状况及其影响因素分析

Analysis of insomnia status of liver transplant recipients and its influencing factors

[目的]探索肝移植受者术后失眠状况及其影响因素。[方法]使用阿森斯失眠量表(AIS)、医院焦虑抑郁量表、领悟社会支持量表测量233例门诊肝移植受者的失眠状况、焦虑及抑郁程度和社会支持程度,并对一般资料进行单因素和多因素Logistic回归分析。[结果]肝移植受者AIS得分为(5.82±4.22)分,83例病人AIS得分>6分,失眠发生率为35.6%。单因素Logistic回归分析显示:性别、体重指数、经济负担、并发症、排异反应、天门冬酸氨基转移酶、焦虑、抑郁、领悟社会支持与失眠有关。多因素Logistic回归分析显示:排异反应、抑郁、领悟社会支持3个变量进入回归方程(P<0.05)。[结论]术后发生排异反应和情感障碍的肝移植受者更容易出现失眠,拥有较高社会支持的受者发生失眠的风险较低,医务人员应重点关注发生排异反应、情感障碍、社会支持较低的肝移植受者的睡眠情况。

Objective. To explore the postoperative insomnia status of liver transplant recipients and its influencing factors. Methods：By using the arsons insomnia scale（AIS）,hospital anxiety and depression scale and perceived social support scale（the insomnia status,anxiety,depression and social support degree） A total of 233 liver transplant recipients in outpatient service were measured on. The general information received the single factor and multi factors Logistic regression analysis. Results：The AIS score of liver transplant recipients was（5.82± 4.22） ,83 cases among them had more than 6,the incidence of insomnia was 35.6%. The single factor Logistic regression analysis showed that gender, BMI, economic burden, complications, rejection, AST, anxiety, depression and perceived social support were related to insomnia. The multivariate Logistic regression analysis showed that 3 variables with rejection,depression and perceived social support entered into the regression equation（P〈 0. 05）. Conclusion： The liver transplant recipients with postoperative rejection and emotional disorders were more prone to insomnia,and liver transplant recipients with higher social support have lower risk of insomnia： Medical personnel should pay more attention on the sleep status of liver transplant recipients with rejection, emotional disorders and lower social support.
