大剂量N-乙酰半胱氨酸治疗158例急性加重期慢性阻塞性肺疾病临床研究 被引量:12

A clinical study on mega-dose N-acetylcysteine in the treatment of 158 patients with acute exacerbation-chronic obstructive pulmonary disease

摘要 目的 探讨大剂量N-乙酰半胱氨酸(NAC)治疗急性加重期慢性阻塞性肺疾病(AE-COPD)的临床疗效。方法选择2015年1~12月我院呼吸内科收治的AE-COPD患者158例,按随机数字表法分为NAC组和对照组各79例,对照组给予常规治疗,NAC组在此基础上给予大剂量NAC治疗。观察两组治疗前后各项观察指标的水平。结果 治疗后NAC组最大峰流速值(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC、血氧分压(PO2)、氧合指数(PO2/Fi O2)、肺顺应性(CL)、血清谷胱甘肽过氧化酶(GSH-PX)、超氧化物歧化酶(SOD)水平均高于对照组,气道阻力(RAW)、气道峰压(PIP)、活性氧类物质(ROS)、丙二醛(MDA)水平均低于对照组,差异有统计学意义(P〈0.05)。NAC组治疗总有效率高于对照组,差异有统计学意义(P〈0.05)。结论 大剂量NAC治疗AE-COPD疗效确切,值得临床应用。 Objective To investigate the clinical effects ofmega-dose N-acetylcysteine (NAC) in the treatment of acute exacerbation-chronic obstructive pulmonary disease (AE-COPD). Methods We selected 158 patients with AE-COPD who were treated in our department from January 2015 to December 2015. The patients were randomly divided into NAC group or control group,79 cases in each group. The control group was given conventional treatment while the NAC group was given NAC treatment in addition to the con- ventional treatment. The various indexes were observed before and after treatment in the both groups. Results After the treatment,levels of peak expiratory flow (PEF) ,forced vital capacity (FVC) ,forced expiratory volume in one second (FEV1), FEV1/FVC, partial pressure of blood oxygen (PO2), oxygenation index (PO2/FiO2, compliance of lung (CL), serum glutathione peroxidase (GSH-PX) and superoxide dismutase (SOD) in the NAC group were higher than those in the control group. However, raw air way resistance (RAW) ,peak airway pressure (PIP) ,reactive oxygen species (ROS) and malondialdehyde (MDA) in the NAC group were lower than that in the control group. The differences were all statistically significant ( all P 〈 0. 05 ). The total effective rate in the NAC group was higher than that in the control group ( P 〈 0. 05 ). Conclusion Mega-dose NAC in the treatment of AE-COPD has obvious curative effect, so it is worthy of clinical application.
