原发性肝癌组织基质金属蛋白酶-9和磷脂酰基醇蛋白聚糖-3表达及临床病理意义 被引量:8

Expression of matrix metalloproteinase-9 protein and glypican-3 protein and the clinicopathological significances in primary hepatic carcinoma

摘要 目的 探讨基质金属蛋白酶-9(MMP-9)和磷脂酰肌醇蛋白聚糖-3(Glypican-3)在原发性肝癌组织中的表达及原发性肝癌患者临床病理特征与两者之间的相关性.方法 收集我院普外科的经病理检查证实的原发性肝癌78例作为实验组(术前未接受放疗和化疗),对照组为22例癌旁组织,采用免疫组织化学法检测癌组织及癌旁组织的MMP-9和Glypican-3的表达,通过电子显微镜高倍镜下评分.结果 MMP-9蛋白在癌组织中阳性表达率为44.7%,表达评分为(1.91±1.59)分,而在癌旁组织未见表达,肝癌组的阳性表达率和表达评分显著高于癌旁组织,差异有统计学意义(P<0.05).Glypican-3蛋白在癌组织及癌旁组织均有表达,但癌组织的表达阳性率及表达评分[57.7%,(2.13±1.75)分]较癌旁组织[9.1%,(0.86±1.18)分]显著增高,且在癌栓形成组中的表达阳性率及表达评分明显高于无癌栓形成组,差异有统计学意义(P<0.05).原发性肝癌组织中MMP-9和Glypican-3蛋白的阳性表达率和表达评分,差异有统计学意义(P<0.01,P<0.05).结论 MMP-9蛋白在癌组织中的高表达,而在癌旁肝脏组织中无明显表达,具有良好的敏感性和特异性,可以作为阴性的肝癌的诊断指标尤其早期肝癌的确诊.Glypican-3蛋白在癌组织中的表达显著高于癌旁肝脏组织。 Objective To analyze the expression of matrix metalloproteinase (MMP)-9 and phosphatidyl inositol proteoglycans-3 (Glypican-3) in primary hepatic carcinoma tissues expression and clinicopathological features of primary liver cancer and the correlation between the two.Methods To collect general surgery of 78 cases with primary hepatic carcinoma confirmed by pathology as the experimental group (did not receive preoperative radiotherapy and chemotherapy) and control group of 22 cases of carcinoma adjacent tissues,the principle of using immunohistochemical of tissue adjacent to carcinoma tissues and MMP-9 and Glypican-3 expression were detected,by electron microscopy at high magnification.Results The MMP-9 protein in cancer tissue protein positive expression rate was 44.7%,for the expression score 1.91 ± 1.59,while no expression in adjacent normal tissue,liver cancer group the positive expression rate and score significantly higher than adjacent tissues,the difference statistically significant,Glypican-3 protein in cancer tissues and adjacent tissues were expressed,but the expression positive rate of cancer and its expression rates (57.7%,2.13 ± 1.75) tissues (9.1%,0.86 ± 1.18) compared with adjacent significantly increased,and the tumor thrombus group expression rate and expression score significantly higher than tumor thrombus group,the difference was statistically significant.Primary hepatocellular carcinoma of MMP-9 and Glypican-3 protein positive expression rate and scores,was statistically significant (P 〈 0.05).Conclusion High expression of MMP-9 protein in the cancer tissue,and no expression in the liver tissue adjacent to carcinoma,has good sensitivity and specificity,the diagnosis of liver cancer can be used as a negative index especially early diagnosis of liver cancer.Glypican-3 protein expression in the tissue of carcinoma of the liver tissue adjacent to carcinoma,is significantly higher than this portends Glypican-3 protein may has the vital significance in the invasion and metastasis of,probably as predictors of malignant degree and poor prognosis of liver cancer.
