鼻咽癌组织中乏氧诱导因子-1α和血管内皮生长因子的表达及其临床意义 被引量:5

摘要 目的研究乏氧诱导因子-1α(hypoxia-inducible factor-1α,HIF-1α)及血管内皮生长因子(vascular endothelial growth factor,VEGF)在鼻咽癌组织中的表达情况,并分析其相关性及与患者预后的关系。方法运用免疫组化染色及评分方法检测56例鼻咽癌患者及18例鼻咽部慢性炎症患者鼻腔黏膜组织中HIF-1α、VEGF的表达差异,并进一步分析HIF-1α、VEGF表达水平与鼻咽癌患者临床病理指标之间的关系。结果免疫组化结果显示,在56例鼻咽癌患者中,HIF-1α蛋白表达阳性率为53.6%(30/56),其中"-"26例,"+"11例,"++"12例,"+++"7例;VEGF蛋白表达阳性率为67.9%(38/56),其中"-"18例,"+"17例,"++"13例,"+++"8例;在18例鼻咽部慢性炎症患者中,HIF-1α和VEGF表达阳性率分别为22.2%(4/18)和27.8%(5/18)。同时,在鼻咽癌患者中,HIF-1α、VEGF的表达均与患者年龄、性别、分化程度无相关性(P>0.05),而与临床分期、T分级、淋巴结转移具有相关性(P<0.05)。Spearman分析显示鼻咽癌组织中HIF-1α表达与VEGF表达呈正相关。结论鼻咽癌组织中存在HIF-1α及VEGF的高表达,HIF-1α及VEGF在鼻咽癌组织中的表达呈正相关,协同促进肿瘤新血管形成。 Objective To detect the expressions of hypoxia-inducible factor-1α( HIF-1α) and vascular endothelial growth factor( VEGF) in nasopharyngeal carcinoma( NPC) tissues,and to investigate their relationships and clinical significances. Methods Immunohistochemistry was used to detect the expression levels of HIF-1α and VEGF in 56 specimens of NPC and 18 of normal nasal mucosal tissue. The relationships between expression of HIF-1α and that of VEGF,their expressions and clinicopathological features of NPC were analyzed. Results In NPC,the positive rate of HIF-1α was 53. 6%( 30 /56),including " + "in 11 cases," + + "in 12 and" + + + "in 7. The positive rate of VEGF was67. 9%( 38 /56) including" + "in 17 cases," + + "in 13,and" + + + "in 8. The positive rates of HIF-1α and VEGF in normal nasal mucosa were 22. 2%( 4 /18) and 27. 8%( 5 /18),respectively. In NPC,the expressions of HIF-1α and VEGF expression had no correlation with patients' age,gender and differentiation degree of tumor( all P〈 0. 05),but were correlated with clinical stage,T stage and lymphatic metastasis of tumor( all P〈 0. 05). Spearman analysis showed that HIF-1α expression was positively correlated with that of VEGF in NPC. Conclusion HIF-1α and VEGF are overexpressed in NPC. Their positively correlated over-expressions may have a synergistic effect on the neovascularization of NPC.
