摘要 随着人口老龄化加剧,颈椎退行性改变及其导致的一系列疾病变得越来越普遍。颈椎退变起源于椎间盘,随着椎间盘退变的进展,颈椎生物力学发生改变,继发骨赘形成、韧带肥厚,最终影响脊髓、神经根等,引发相应的临床症状[1]。MRI不仅能清楚显示椎间盘、韧带等软组织结构的退行性改变,还能反映脊髓、神经等受压情况,是检查颈椎退变的主要影像学方法之一。然而,目前常规颈椎MRI是在仰卧体位下行颈椎中立位扫描,只能获得非承重状态的静态图像,而颈椎的日常状态多处于承重体位,伴随着复杂的动态姿势改变。故常规颈椎MRI不能完全真实反映日常状态下颈椎生理负荷情况,且忽略了椎管、椎间盘、韧带等各颈椎结构对脊髓的动态压迫性改变,以及退变结构对颈椎动力学的改变。因此,近年来不少学者开始利用动态MRI研究颈椎退变,包括退变对颈椎动力学的影响,以及颈椎屈伸活动时退变结构的动态变化,重点是研究脊髓的动态受压情况及其临床意义。现将相关研究进展综述如下。
