乳腺癌超声及超声造影表现与生物学预后因子的相关性

Correlation of features of breast cancer on ultrasound and biological prognostic factors

目的探讨乳腺癌超声及超声造影表现特征与其生物学预后因子之间的相关性。方法对239例乳腺癌患者术前行超声及超声造影检查;术后检测雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子受体2(HER-2)和Ki-67表达,分析其与超声及超声造影表现的相关性。结果肿块边缘、钙化、周边回声晕、肿块形态及超声造影增强方式与ER、PR、HER-2及Ki-67表达无明显相关性(P>0.05)。乳腺癌后方回声衰减是ER阳性表达的预测因素;乳腺癌直径>2cm是HER-2和Ki-67阳性表达的预测因素。乳腺癌超声造影表现为高增强和晚消退是HER-2阳性表达的预测因素。结论乳腺癌超声及超声造影表现与血清ER、PR、HER-2和Ki-67表达之间存在一定相关性。

Objective To explore the correlation between breast cancer ultrasound and contrastenhanced ultrasound features and biological prognostic factors.Methods Ultrasonography and contrast-enhanced ultrasound were carried out in 239 patients with breast cancer.Serum expressions of estrogen receptor（ER）,progesterone receptor（PR）,human epidermal growth factor receptor 2（HER-2）and Ki-67 after operation were detected.The correlation of breast cancer ultrasound and contrast-enhanced ultrasound features and biological prognostic factors was analyzed.Results The tumor margin of breast cancer,calcification,hyperechoic halo,tumor shape and contrast-enhanced ultrasound had no relation to the expressions of ER,PR,HER-2and Ki-67（P〉0.05）.Posterior acoustic attenuation of breast cancer was the predictor for ER positive expression.Breast cancer〉 2cm in diameter was the predictor for HER-2and Ki-67 positive expressions.The highly enhanced and lately faded breast cancer on contrast-enhanced ultrasound were the predictors for HER-2positive expression.Conclusion There is a certain correlation between the expressions of ER,PR,HER-2and Ki-67 and the features of breast cancer on ultrasound and contrast-enhanced ultrasound.
