摘要
目的:研究桥本氏甲状腺炎患者的甲状腺超声弹性对比指数(ECI)与血清自身抗体、Th1/Th2型细胞因子含量的关系。方法:选择在我院确诊为桥本氏甲状腺炎(HT)的68例患者纳作为HT组,体检的健康志愿者60例作为对照组,行超声检查并测定ECI,取血清并测定抗甲状腺过氧化物酶抗体(TPO-Ab)、抗甲状腺球蛋白抗体(TG-Ab)、γ-干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)、白介素-2(IL-2)、白介素-4(IL-4)、白介素-10(IL-10)的含量,取外周血并测定CD30和CD195阳性表达的百分率。结果:HT组患者的ECI显著高于对照组且微小结节型HT患者的ECI显著低于网格型HT患者(P<0.05);HT组患者血清中TPO-Ab、TG-Ab、IFN-γ、TNF-α、IL-2的含量以及外周血中CD195的阳性表达率显著高于对照组(P<0.05),IL-4、IL-10的含量以及外周血中CD30的阳性表达率显著低于对照组(P<0.05);微小结节型HT患者血清中TPO-Ab、TG-Ab、IFN-γ、TNF-α、IL-2的含量以及外周血中CD195的阳性表达率显著低于网格型HT患者(P<0.05),IL-4、IL-10的含量以及外周血中CD30的阳性表达率显著高于网格型HT患者(P<0.05);ECI与TPO-Ab、TG-Ab、IFN-γ、TNF-α、IL-2b、CD195呈正相关(P<0.05),与IL-4、IL-10、CD30呈负相关(P<0.05)。结论:桥本氏甲状腺炎患者的ECL显著升高且能够用于免疫功能紊乱程度的评估。
Objective: To study the relationship of thyroid ultrasound elasticity contrast index (ECI) with serum au- toantibody and Th1/Th2 cytokine levels in patients with Hashimoto's thyroiditis. Methods: A total of 68 patients diagnosed with Hashimoto's thyroiditis (HT) in our hospitalas were selected as HT group and 60 healthy volunteers were selected as con- trol group ; ultrasound examination was performed to determine ECL, serum was collected to determine anti-thyroid peroxidase antibody (TPO-Ab), anti-thyroglobulin antibodies (TG-Ab), IFN-γ, TNF-α, IL-2, IL-4 and IL-10 levels, and peripheralblood was collected to determine the positive expression rate of CD30 and CD195. Results: ECI of HT group was significantly higher than that of control group and the ECI of patients with small nodule type HT was significantly lower than that of pa- tients with grid type HT (P〈0.05) ; TPO-Ab, TG-Ab, IFN-γ, TNF-α and IL-2 levels in serum and positive expression rate of CD195 in peripheral blood of HT group were significantly higher than those of control group while IL-4 and IL-IO levels (P 〈0.05) and positive expression rate of CD30 in peripheral blood were significantly lower than those of control group (P〈0. 05) ; TPO-Ab, TG-Ab, IFN-γ, TNF-α and IL-2 levels in serum and positive expression rate of CD195 in peripheral blood of patients with small nodule type HT were significantly lower than those of patients with grid type HT while IL-4 (P(0.05) and IL-10 levels and positive expression rate of CD30 in peripheral blood were significantly higher than those of patients with grid type HT (P(0.05) ; ECI was positively correlated with TPO-Ab, TG-Ab, IFN-γ, TNF-α, IL-2b and CD195, and negatively correlated with IL-4, IL-10 and CD30 (P〈0.05). Conclusiens: ECL significantly increases in patients with Hashimoto's thy- roiditis and it can be used to evaluate the degree of immune dysfunction.
