复方苦参注射液IL-2及干扰素辅助治疗肺癌伴恶性胸腔积液疗效和不良反应

The curative effect and adverse reactions of Composite Radix Sophora Flavescentis Injection,IL-2 and IFN-α in lung cancer complicated with malignant pleural effusion

目的观察复方苦参注射液、白介素-2(IL-2)及干扰素-α(IFN-α)辅助治疗肺癌伴恶性胸腔积液的临床疗效及不良反应。方法纳入肺癌伴恶性胸腔积液患者70例,根据治疗方案的不同分为研究组与对照组。对照组在患者的胸腔内注射IL-2与IFN-α,研究组在对照组的治疗基础上加入复方苦参注射液。观察两组患者近期疗效以及治疗期间的不良反应情况;观察治疗前后两组患者的免疫功能情况。结果研究组治疗有效率为91.43%,对照组治疗有效率为71.43%,组间差异有统计学意义(P<0.05);治疗前,两组患者CD3+、CD4+、CD8+、CD4+/CD8+水平差异均无统计学意义(P>0.05);治疗后研究组CD3+、CD4+、CD8+、CD4+/CD8+水平分别为(63.20±10.56)%、(55.02±14.15)%、(38.18±10.74)%和(1.59±0.72)%,对照组分别为(58.78±9.61)%、(49.18±11.06)%、(40.62±10.49)%和(1.33±0.56)%,组间差异均有统计学意义(P<0.05);研究组治疗期间发热、胸痛、胃肠道反应发生率分别为34.29%、5.71%和8.57%,对照组分别为60.00%、22.86%和28.57%,组间差异均有统计学意义(P<0.05)。结论复方苦参注射液、IL-2及IFN-α辅助治疗肺癌伴恶性胸腔积液的临床疗效较好,能够显著改善患者的免疫功能,且不良反应发生率较低。

Objective To review the curative effect and adverse reactions of adjuvant therapy of Composite Radix Sophora Flavescentis Injection, IL-2 and IFN-a in lung cancer complicated with malignant pleural effusion. Methods 70 patients of lung cancer complicated with malignant pleural effusion were divided into control group and study group. The control group was treated with the injection of IL-2 and IFN-a. On the basis of control group, the study group was treated with Composite Radix Sophora Flavescentis Injection. The short-term clinical effect and adverse reactions as well as immune function before and after treatment for two groups were observed. Results The effective rate of study group （91.43%） was significantly higher than that of control group （71.43%） （P〈0.05）. Before treatment, the level of CD3＋ , CD4＋ , CD8＋ , CD4＋/CD8＋ for the two groups were not significantly different （P〉0.05）. After treatment, the level of CD3＋ , CD4＋ , CD8＋ , CD4＋/CD8＋ for study group were （63.20±10.56）%, （55.02 ±14.15） %, （38.18 ±10.74）% and （1.59±0.72）. The level for control group were （58.78±9.61）%, （49.18±11.06）%, （40.62± 10.49）%and （1.33±0.56） （P〈0.05）. The incidence rate of fever, chest pain and gastrointestinal reaction for study group was 34.29%, 5.71% and 8.57%. The rate for control group was 60.00%, 22.86% and 28.57% （P〈0.05）. Conclusion The adjuvant therapy of Composite Radix Sophora Flavescentis Injection, IL-2 and IFN-a has a good effect on treatment of lung cancer complicated with malignant pleural effusion, which can greatly improve the patient＇s immune function and have a lower adverse event rate.
