肝硬化再生结节患者的核磁共振诊断分析

Diagnostic analysis of nuclear magnetic resonance in patients with cirrhosis regenerative nodule

目的研究分析核磁共振用于肝硬化再生结节的诊断价值。方法随机选取2014年1月～2016年3月本院收治的肝硬化再生结节患者46例进行观察，分析本组患者核磁共振检查结果及其确诊率，总结肝硬化再生结节核磁共振的特点及其诊断价值。结果45例通过核磁共振诊断为肝硬化再生结节，诊断率为97．83％。常规扫描：脂肪抑制T1WI多表现为稍高或等信号，T2WI大多为低信号，增强扫描：门静脉期、延迟期与强化肝实质比较为低信号结节，动脉期未见强化，癌结节T1WI多表现为稍低或等信号，个别可见稍高信号，T2WI多表现为较高信号，增强扫描可见动脉期明显强化，门静脉期和延迟期强化减弱。结论核磁共振用于肝硬化再生结节临床诊断，能够早期鉴别良恶性疾病，同时还能对恶性结节临床疗效进行评价，值得临床参考应用。

Objective To study the diagnostic value of nuclear magnetic resonance in cirrhosis regenerative nodules. Methods 46 patients with cirrhosis regenerative nodules cured in our hospital from January 2014 to March 2016 were randomly selected and observed to analyze nuclear magnetic resonance examination results and its diagnosis rate, and to summarize the characteristics and diagnostic value of nuclear magnetic resonance of the regenerative nodule of liver cirrhosis. Results 45 cases were diagnosed as cirrhosis regenerative nodule by nuclear magnetic resonance, and the diagnostic rate was 97.83%. Routine scan： Fat suppression T1WI mostly showed slightly higher or equal signal, and T2WI mostly showed low signal. Enhanced scan： Portal venous phase, delayed phase and enhanced liver parenchyma were compared to low signal nodules, there was no enhancement in the arterial phase, cancer nodules T1WI mostly showed slightly low or equal signal with slightly higher signal, T2WI mostly showed high signal, enhanced scanning showed obvious enhancement of arterial phase, and enhancement weakened portal venous phase and delayed phase. Conclusion Nuclear magnetic resonance in diagnosis of cirrhosis regenerative nodules is able to identify early benign and malignant diseases. At the same time, it can evaluate the clinical efficacy of malignant nodules. It is worthy of clinical reference application.
