股骨颈骨折术后内固定取出时间对髋关节功能影响的临床观察

目的探讨股骨颈骨折术后内固定物取出时间对髋关节功能的影响及确定何时去除内固定较好。方法：选择行单侧股骨颈骨折空心加压螺钉内固定术后内固定取出术的患者81例，男42例，女39例。A组：内固定在体内存留1-2年30例：B组：内固定存留2-3年23例；C组；3年及以上的28例。术后经3～5年随访，通过Harris髋关节评分和髋关节功能评定评分比较，评分应用SPASS16．0软件进行数据分析。结果：1～2年内固定物取出与2～3年、3年以上内固定取出后髋关节活动评分及股骨头坏死发生率差异均有统计学意义：2～3年、3年以上内固定取出后两组相互比较差异无统计学意义。结论：股骨颈骨折术后内固定取出时间在2—3年更为合适，有以利于提高髋关节活动，降低股骨头坏死的发生率可能。

Objective To explore the influence of the time of internal fixation retaining in body after femoral neck fractures operation on hip joint fnnetion and when to remove the internal fixation is best. Methods Choose 81 cases which include 42 male cases and 39 female cases from the hospitalized cases doing removing operation of hollow compression screws for internal fixation in unilateral femoral neck fracture treatment between May 2010 and May 2013 .Group A： 30 cases of the internal fixation retaining one to two years in their bodies; Group B： 23 cases of the internal fixation retaining two to three years in their bodies; Group C： 28 cases of the internal fixation retaining more than three years in their bodies. The removing operation is followed-up for three to five years and compared through Harris hip score and hip joint function score whose data is analyzed by SPASS13.0 software. Results The differences of hip function score and femoral head necrosis condition between the removing operation of internal fixation retaining one to two years. two to three years and more than three years, all have statistical significance; the differences of comparing the removing operation of internal fixation retaining two to three years and more than three years, doesn＇t have statistical significance. Conclusions The removing operation of internal fixation being done after retaining more than two years in their bodies is more suitable for patients, which could improve hip range of activities and reduce the occurrence of femoral head necrosis.
