摘要
目的研究降钙素原在心脏瓣膜置换术后感染早期诊断的价值。方法:选取在体外循环下行心脏瓣膜置换术后发生感染的病例16例为观察组(A):5例为主动脉瓣置换,7例为二尖瓣置换,4为双瓣置换:选取同期在我院行心脏瓣膜置换未发生感染的患者共64例为对照组(B),行1:4配对比较(年龄、性别、CPB时间及病情类似);对两组降钙素原血浓度在术前、术后24 h、48 h、72 h进行监测,并进行组间、组内进行比较。结果:所有患者术后降钙素原均较前升高,差异有统计学意义(P〈0.05);观察组中术后降钙素原下降比较缓慢,且有第二高峰存在;对照组中术后检测降钙素原浓度曾明显下降趋势,术后第三天接近正常值水平。结论:降钙素原在诊断心脏瓣膜置换术后术后早期感染具有重要的意义。
Objective Research the diagnostic value of procalcitonin after cardiac surgery bacterial infection. Methods: From January 2013 to December 2015 , 16patients in department of cardiothoracic surgery of Huaian First People's Hospital infection occurs after cardiac surgery in cardiac valve replacement in the obser-vation group (A); Including 5 cases of aortic valve replacement, mitral valve replacement in 7 cases, 4 double valve replacement. 64 patients uninfected after car-diac surgery in cardiac valve replacement in the control group (B) during the same time. They were matched with Control group by the same sex, age,CPB tine,andDisease. The PCT status of blood sugar, before surgery 24h, after 24h, 48h, 72h monitoring, were compared between two groups. Results: The PCT were elevatedthan Preoperative in all patients , the difference was statistically significant (P〉 0.05);they were decreased more slowly in observation group; they were decreasedsignificantly in observation group;they closed to the normal level after the third day. Conclusion:the procalcitonin in blood status were important significancein indiagnosis cardiac surgery early infection in cardiac valve replacement.
