临床药师对临床不合理用药医嘱实施药学干预的研究

目的研究临床药师在临床不合理用药医嘱药学干预中发挥的作用。方法：选择2014年8月-2015年8月本院不合理用药医嘱的临床药师干预记录82条，临床病例68例，针对不合理用药医嘱相关药物种类进行分析，了解相关药学问题，同时统计患者对医师干预的接受状况等。结果：临床药师药学干预涉及到的大部分为专科用药，其构成比为84．296，专业用药干预成功率为81．6％；辅助治疗药物构成比为4．996，干预成功率为87．5％。临床药师药学干预过程中涉及到的药学问题主要为不良反应处理问题以及联合用药问题，分别占56．1％和26．8％，干预成功率分别为92．5％和61．4％。结论：针对不合理用药医嘱进行药学干预的过程中，临床药师能够通过查房发现不合理用药医嘱，通过适当的干预可以促进不合理用药现象的减少。

Objective Clinical pharmacists play a role in the clinical irrational drug prescription pharmaceutical interventions. Methods：Choose from August 2014 to August 2015 in our hospital clinical pharmacist medication orders unreasonable intervention record 82, clinicalcases, 68 cases were analyzed for irrational medication orders related tO the types of drugs, understand the relevant pharmaceutical problems,while statistics patient physician acceptance and other interventions.Results：Clinical pharmacist pharmacy intervention involving the majority ofspecialty drugs, which constitute more than 84.2%, professional drug intervention success rate of 81.6%; secondary treatment constitute morethan 4,9%, the intervention success rate was 87.5%. Clinical pharmacist pharmacy intervention process involved in pharmaceutical issuesmainly deal with the problem of adverse reactions as well as the combination problem, accounting for 56.1% and 26.8%, respectively, thesuccess rate of the intervention was 92.5% and 61.4%, respectively. Conclusion： Process for pharmaceutical intervention irrational drugprescription, the clinical pharmacist medication orders can be found unreasonable by rounds, through appropriate interventions could contributeto redm-ing the pheo0menon of irrational drug.
