结核性胸膜炎血清和胸水中IL-27的表达水平及其临床意义

目的探讨结核性胸膜炎血清和胸水中IL-27的表达水平及其临床意义。方法：收治结核性胸膜炎患者30例和非结核性胸膜炎患者30例。另选择健康体检人员30例为正常对照组。采用双抗体夹心ELISA测定血清及胸水中IL-27的表达水平。结果：结核性胸膜炎组血清中IL-27的表达水平为（41．2±10．5）ng／L，高于正常对照组和非结核性胸膜炎组（均P〈0．05）。结核性胸膜炎组胸水中IL-27的表达水平为（379．6±57．6）ng／L，高于非结核性胸膜炎组（P（O．05）。结核性胸膜炎组患者在规则治疗1、3、7、14 d时，血清和胸水中IL-27的表达水平均逐渐下降，血清中IL-27的表达于治疗3d时显著低于入院时水平（P〈0．05），胸水中IL-27的表达于治疗7 d时显著低于入院时水平（P〉O．05）。结论：IL-27可以作为结核性胸膜炎早期诊断及观察治疗效果的分子学免疫指标，值得临床推广。

Objective To investigate expression level of IL-27 in the serum and chest fluid in patients with tuberculous pleurisy （TP）and itsclinical significance. Methods： From June 2014 to October 2015, 30 patients with TP and 30 patients without TP were collected in our hospital.Moreover,30 healthy physical examination personnel were recruited as control. ELISA was used to measure level of IL-27 in the serum and thechest fluid from 30 patients with TP,30 other patients without TP and 30 healthy physical examination personnel. Results： The expression levelof IL-27 in the serum were 41,2±10.5 ng/L in patients with TP, which was higher than that in patients without TP and healthy physicalexamination personnel （P〈0.05）. The expression level of IL-27 in the chest fluid were 379.6 ±57.6 ng/L in patients with TP, which was higherthan that in patients without TP（P〈0.05）. After treatment for 1, 3, 7and 14 days, the level of IL-27 in the serum and chest fluid were obviouslydecreased, compared with those at the admission in hospital （P〈0.05）.The level of IL-27 in the serum was decreased from 3 days after thetreatment, and the level of IL-27 in the chest fluid was lower from 7 days after the treatment.There were significant differences （P〈0.05）.Conclusion：IL-27 can be an index of molecular immunology to early diagnosis and judgement of therapeutic effect for riP disease. It would bevaluable in clinic use.
