老年患者胃黏膜不同病理形态幽门螺杆菌感染相关性研究

Pathological studies of gastric mucosa infections with Helicobacter pylori in elderly patients

目的研究老年患者胃黏膜不同形态病理幽门螺杆菌感染的相关性,为临床诊治提供依据。方法选取2015年1月-2016年1月医院收治的219例胃黏膜活检老年患者,采用不同染色方法观察幽门螺杆菌感染与老年患者胃黏膜形态病理学变化及相关性;采用SPSS18.0软件进行统计分析。结果 219例老年胃黏膜活检患者中,124例检测出幽门螺杆菌,感染率为56.62%;其中,92例病理呈现螺旋状伴球形幽门螺杆菌、32例单纯球形幽门螺杆菌;单因素显示与炎症程度、间质淋巴数量以及异性增生关系密切(P<0.05);非条件logistic分析显示,幽门螺杆菌感染与老年患者性别、炎症程度、间质淋巴数量以及异性增生关系密切(P<0.05)。结论球形幽门螺杆菌与螺旋状幽门螺杆菌均能诱发胃黏膜感染,且螺旋状幽门螺杆菌致病性强于球形幽门螺杆菌,应加强检测,提高临床治愈率。

OBJECTIVE To explore the correlation between the different pathological forms of gastric mucosa infections with Helicobacter pylori in elderly patients so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 219 elderly patients who received the gastric mucosa biopsy in the hospital from Jan 2015 to Jan 2016 were enrolled in the study,and the relationship between the H.pylori infection and pathological change of gastric mucosa was observed by using different staining methods.RESULTS Of the 219 elderly patients who underwent the gastric mucosa biopsy,124 were detected positive for H.pylori,with the infection rate 56.62%;the pathological analysis showed that 92 patients presented with mixed spiral and spherical H.pylori,and 32 patients presented with single spherical H.pylori.The univariate analysis indicated that the H.pylori infection was closely associated with the degree of inflammation,number of interstitial lymphoids,and heterosexual hyperplasia（P〈0.05）.The non-conditional logistic analysis showed that the H.pylori infections was closely associated with the gender,degree of inflammation,number of interstitial lymphoids,and heterosexual hyperplasia（P〈0.05）.CONCLUSIONBoth spherical H.pylori and spiral H.pylori can induce the gastric mucosa infections,and the pathogenicity of the spiral H.pylori is greater than that of the spherical H.pylori.It is necessary to strengthen the detection so as to raise the clinical cure rate.
