传染病医院全自动血细胞分析仪复检规则的建立

Establishment of review criteria of automatic hematology analyzer in infectious disease hospital

目的:参考Sysmex XE-2100血细胞分析仪复检标准,依据国内专家组推荐的23条复检规则,通过对试验数据进行分析,制定适合传染病医院使用的血细胞复检规则。方法:随机选取704份医院患者血液标本,检测血常规的同时涂片镜检。以专家组推荐的23条复检规则为评估基础,根据传染病医院病种特点,优化相应复检规则,以涂片镜检为金标准,分别计算出血细胞的真阳性率、假阳性率、真阴性率、假阴性率和涂片复检率。结果:初始复检规则评估结果真阳性率10.93%,假阳性率42.05%,真阴性率44.60%,假阴性率2.41%;涂片复检率52.98%。第1次优化后结果真阳性率9.80%,假阳性率19.89%,真阴性率67.19%,假阴性率3.13%;涂片复检率29.69%。第2次优化后结果真阳性率9.23%,假阳性率16.05%,真阴性率71.16%,假阴性率3.55%;涂片复检率25.28%。最终评估结果涂片复检率<30%,假阴性率均<5.0%,且不漏检白血病细胞。结论:初步建立的适用于传染病医院实验室全自动血细胞分析仪复检规则,可有效提高血细胞检验工作效率。血细胞复检规则应在血细胞检验工作中不断改进和完善。

Objective Through analysis of the laboratory data to establish the suitablehematology review criteria for infectious disease hospital according to 23 review criteriarecommended by experts group and formulation review criteria by Sysmex XE-2100 hematologyanalyzer. Methods： The total 704 patient samples were randomly detected and blood smear was reviewed. Based onthe review criteria from the experts group, we evaluated the true positive ratio （TP）, the false positive ratio （FP）, thetrue negative ratio （TN）, the false negative ratio （FN） and ratio of smear review according to the disease characteristicsof infectious disease hospital. Results： The assessment results of initial review criteria were followed： TP was 10.93%,FP was 42.05%, TN was 44.60%, FN was 2.41%, and review rate was 52.98%; after first optimizing review criteria,TP was 9.80%, FP was 19.89%,TN was 67.19%, FN was 3.13%, and review rate was 29.69%; after second optimizingreview criteria, TP was 9.23%, FP was 16.05%, TN was 71.16%, FN was 3.55%, and review rate was 25.28%. The finalassessment results showed review rate was less than 30%, FN was less than 5.0%, and no leukemia cells was missed.Conclusion： We established the suitable hematology review criteria for infectious disease hospital and improved workefficiency. The review criteria should be constantly improved in hematology analysis.
