综合护理干预对COPD患者疾病应对态度及生活质量的影响

Effect of comprehensive nursing intervention on response attitude to disease and quality of life in COPD patients

目的探讨综合护理干预对慢性阻塞性肺疾病(COPD)患者疾病应对态度及生活质量的影响。方法 112例COPD患者随机分为观察组和对照组,各56例。对照组常规护理,观察组在对照组基础上实施综合护理干预。采用Mishel疾病不确定感量表(MUIS)和Herth希望量表(HHI)评价患者对疾病的应对态度,采用焦虑自评量表(SAS)评估患者焦虑状态,采用简易健康调查问卷(SF-36)评估患者生活质量。结果干预后观察组MUIS评分低于对照组,HHI评分高于对照组,差异有统计学意义(P<0.01)。SAS评分结果显示,观察组重度焦虑患者比例显著低于对照组(P<0.05)。观察组SF-36量表中情感职能、心理状态、生理机能和社会关系得分均显著高于对照组(P<0.01)。结论综合护理干预能有效改善COPD患者疾病应对态度、提高生活质量。

Objective To investigate the effect of comprehensive nursing intervention on response attitude to disease and quality of life in patients with chronic obstructive pulmonary disease （COPD） . Methods A total of 112 COPD patients were randomly divided into the observation group and the control group, with 56 cases in each group. Both two groups were given routine nursing care and the observation group was applied with extra comprehensive nursing intervention. The response attitude to disease was assessed by Mishel uncertainty in illness scale （ MUIS） and Herth hope index （HHI） . The mental status of patients was assessed by self-rating anxiety scale （SAS） . The quality of life was assessed by 36-item short-form health survey questionnaire （SF-36）. Results The observation group had a lower MUIS and a higher HHI compared with the control group after intervention （ P 〈0.01） . The proportion of patient with severe anxiety according to the SAS was significant lower in observation group than that of controls （ P 〈0. 05） . Scores of role emotion, mental health, physiology function and social relation in SF-36 were higher in the observation group than those in the control group （ P 〈 0 .0 1 ） . Conclusion Comprehensive nursing intervention can improve the response attitude to disease and quality of life in COPD patients.
