中医分期辨治放射性肺炎

Staging Treatment of Traditional Chinese Medicine Syndrome Differentiation of Radioactive Pneumonia

中医对于放射性肺炎的治疗,急性期治以解毒养阴,慢性期治以活血通络,活血通络应贯穿始终,因为放射线属阳热毒邪,在早期即可灼伤肺之络脉,造成络脉不通而形成瘀血,同时,瘀血又可进一步阻滞气机,形成留瘀化热、血瘀生水之恶性循环,从而发展为湿、毒、瘀互结之证。因此,本病一旦发生,其病理演变必向血瘀方向发展。急性期和慢性期的基本方中皆有活血之丹参,原因就在于此。其次,临证用药应时刻注意固护胃气,急性期的清热解毒之药苦寒败胃,养阴之药滋腻碍胃,慢性期的活血药攻伐伤胃,都可损伤脾胃,造成胃气受损。中医以脾胃为后天之本,气血生化之源,脾胃一败,诸药难投。

In terms of treating radioactive pneumonia according to TCM,method of removing toxin and nourishing yin should be used at the acute stage; Method of activating blood circulation and dredging meridians should be used at the chronic stage,which should go through the whole treating process,because the radiation belongs to yang- heat- toxin evil,which may burn the collaterals of lung at the early stage,giving rise to blood stasis due to obstructed meridians. At the same time,blood stasis may further block qi activity,form the retention of heat and blood stasis,and generate water,which is a vicious cycle and may develop into syndrome of intermingled dampness,toxin and stasis. So,once the disease appears,its pathological evolution will be to develop in the direction of blood stasis. That is why Danshen（ Radix et Rhizoma Salviae Miltiorrhizae） is included at both acute and chronic stages. Second,clinical medication should pay attention to protecting stomach qi,because the herbs with functions of clearing away heat and removing toxin at the acute stage may destroy the stomach; The herbs with functions of nourishing yin may obstruct the stomach; The herbs with functions of activating blood circulation at the chronic stage may attack the stomach. All the above may damage the spleen and stomach qi. The spleen and stomach are acquired bases and vital genesis of qi and blood. If they are destroyed,any medicine will be useless.
