孕晚期彩超脐血流S/D比值在高危妊娠结局预测中的价值分析

The Value Clinical Effect of Umbilical Cord Blood Flow S/D Ratio in the Late Pregnancy and the Prediction of High Risk Pregnancy Outcome

目的:探讨高危妊娠孕晚期监测脐血流S/D比值在预测妊娠结局中的应用。方法:回顾性分析2014年10月-2015年10月本院收治的120例晚期妊娠孕产妇的临床资料,根据妊娠状态的不同分为高危妊娠组和正常妊娠组,每组60例。应用彩色多普勒超声对两组孕产妇的子宫螺旋动脉血流进行检测,分别测量其收缩期峰值流速/舒张末期流速(S/D)、搏动指数(PI)及阻力指数(RI)等血流参数。结果:高危妊娠组孕妇子宫螺旋动脉S/D、PI和RI值分别为(1.84±0.14)、(0.37±0.08)和(0.48±0.04),明显高于正常妊娠组的(1.52±0.08)、(0.32±0.04)和(0.36±0.06),比较差异均有统计学意义(P<0.05);高危妊娠组中子宫螺旋动脉S/D值≥1.7的宫内窘迫、生长受限和新生儿窒息的发生率明显高于子宫螺旋动脉S/D值<1.7,比较差异均有统计学意义(P<0.05)。结论:孕晚期彩色多普勒超声检测脐血流S/D比值可作为高危妊娠者预测妊娠结局的参数之一,具有一定的临床价值。

Objective： To investigate the application value of the umbilical cord blood flow S/D ratio detected by color Doppler ultrasound in predicting high-rick pregnancy outcome.Method： From October 2014 to October 2015, 120 cases of late pregnancy in our hospital were retrospective analyzed, they were divided into the high-rick pregnancy group and the normal pregnancy group according to different pregnancy conditions, each group had 60 cases.The systolic/diastolic （ S/D ）, pulsatility index （ PI ） and resistance index （ RI ） in uterine spiral arteries of two groups were detected by color Doppler ultrasound.Result ： The uterine spiral arteries values of S/D, PI and R1 were respectively （ 1.84 ± 0.14 ）, （ 0.37 ± 0.08 ） and （ 0.48 ± 0.04 ） in high-rick pregnancy group while the normal pregnancy group were respectively （ 1.52 ±0.08 ）, （ 0.32 ± 0.04 ） and （ 0.36 ± 0.06 ）, the differences were statistically significant （ P〈0.05 ）, the incidence rate of fetal distress, fetal growth restriction and neonatal asphyxia in high-rick pregnancy group were notably increased when S/D 1〉 1.7, which were notably more than that when S/D〈1.7, the differences were statistically significant （P〈0.05） .Conclusion： Detection of S/D ratio in umbilical cord blood flow by color Doppler ultrasound in the late pregnancy can be used as one of the parameters for predicting the outcome of pregnancy, which has certain clinical value.
