直肌返折术治疗水平斜视60例临床研究

Clinical Research on the Treatment of Horizontal Strabismus by Rectus Turn-back Operation in 60 Cases

目的探讨直肌折返术治疗水平斜视的临床效果.方法选取60例共同性斜视患者,随机分成2组,A组行直肌返折术,B组行直肌截除术,比较两组术后眼正位率、术中及术后并发症情况.结果两组术后1周、1个月、1 a眼正位率之间比较差异无统计学意义(P>0.05).术中并发症眼心反射发生率A组为3.3%,优于B组(40%),角膜上皮剥脱发生率A组为6.67%,优于B组(40%),两者比较差异均具有统计学意义.术后结膜血肿发生率A组为0,明显优于B组(6.67%),两者比较具有统计学意义.结论直肌折返术同直肌截除术相比具有相同的手术效果,操作简单,损伤小,术中及术后并发症明显降低.

Objective To investigate the clinical effect of rectus turn-back operation on the treatment of horizontal strabismus. Method A total of 60 patients with concomitant strabismus were randomly divided into 2 groups. The patients in group A line accepted rectusturn-back operation, and group B was treated with rectus resection. The correction rate, intra-operative and postoperative complications were compared between the two groups. Results There was no significant difference in correct position rate between the two groups at 1 week, 1 month and 1 year postoperation （ P〉0.05 ）. The incidences of intra-operative complications oculocardiac reflex and corneal epithelial exfoliation were 3.3 % and 6.67 % in group A,respectively,which were statistically lower than the rates in group B （40%）. No case of conjunctival hematoma was observed in group A,and the incidence in group B was 6.67% , suggesting statistically significant difference. Conclusion Rectus turn-back surgery has the same operative results with rectus resection. But the former surgery can ease the operation, lessen damage and obviously decrease the incidences of intra-operative and postoperative complications.
