经内镜5-FU缓释化疗粒子治疗晚期上消化道肿瘤的体会

Clinical curative effect of awake anesthesia via endoscopic 5-FU slow-release chemotherapy of particles on the treatment of upper digestive tract tumor

目的：探讨患者在清醒麻醉下,经内镜5-氟尿嘧啶（5-FU）缓释化疗粒子治疗晚期上消化道肿瘤的临床疗效优势近期疗效。方法：37例上消化道肿瘤晚期患者,肿瘤均为隆起型腺癌,患者均未进行手术或放化疗处理。在内镜直视下,将5-FU粒子密集或多点植入肿瘤结节及其周围浸润组织,平均注射45~60粒（约120~150mg）。结果：37例晚期上消化道肿瘤患者有不同程度临床症状缓解,术后1~2个月随访内镜显示,33例（89.2%）肿块有程度不等的缩小、局限、平坦,消化道梗阻减轻。术后患者血常规、肝肾功能检查无明显异常,未见出血、穿孔等并发症发生。结论：内镜直视下,植入缓释5-FU粒子化疗,可明显改善晚期上消化道肿瘤患者的临床症状,限制肿瘤生长,且不良反应不明显,具有一定的临床疗效优势。

Objective：To investigate the near future clinical curative effect of awake anesthesia via endoscopic 5-FU slow-release chemotherapy of particles on the treatment of upper digestive tract tumor patients.Methods：37 patients with late stage of upper digestive tract tumor,adenocarcinoma tumors were protrude type,patients did not undergo surgery or radiation and chemotherapy treatment.Under endoscopy,5-FU particle dense or multipoint and around tumor nodules infiltration transplanted tissue,average 45 to 60 injection（about 120~150 mg）.Results：in 37 cases of late stage upper digestive tract cancer patients had different degree of clinical symptoms,postoperative 1~2 months followup endoscopy showed that 33 cases（89.2%） mass with varying degrees of narrow,limited,flat,alleviate digestive tract obstruction.Routine blood,liver and kidney function in patients with postoperative examination had no obvious abnormalities,no complications such as hemorrhage,perforation occurred.Conclusion：endoscopic direct,implantation of slow-release chemotherapy,5-Fu particles can significantly improve the clinical symptoms of patients with late upper digestive tract tumor,the clinical curative effect of late digestive tract tumor has a certain advantage.
