屈螺酮炔雌醇片治疗流产后宫内残留的疗效分析

Curative effect analysis of drospirenone and ethinylestradiol tablets for post-abortion intrauterine residue

目的探讨屈螺酮炔雌醇片治疗流产后宫内残留的效果。方法选择我院收治的人工流产、药物流产、自然流产等各种流产后仍有少量阴道流血,B超证实宫腔内有少量残留(残留物最大直径≤35 mm)且血清中β-HCG≤500.00 m IU/m L的患者共100例,按照区组随机化的原则分为观察组与对照组,观察组50例口服屈螺酮炔雌醇片,对照组50例口服米非司酮片,观察两组治疗后阴道流血持续时间及B超检查宫内残留消失的情况。结果观察组阴道流血持续时间为(4.31±1.23)d,对照组阴道流血持续时间(8.31±1.91)d,差异有统计学意义(P<0.05);观察组阴道流血的止血率100%,对照组阴道流血停止率80%,差异有统计学意义(P<0.05);经过B超检查,观察组的宫腔残留治愈率96%,对照组的宫腔残留治愈率62%,差异有统计学意义(P<0.05)。结论屈螺酮炔雌醇片治疗流产后宫内残留的效果远优于米非司酮片,其疗效确切,避免清宫术的同时,也起到了保护子宫内膜作用,值得临床推广。

Objective To explore the effectiveness of drospirenone and ethinylestradiol tablets in treatment of post- abortion intrauterine residue. Methods A total of I00 post-abortion （including induced abortion,medical abortion and spontaneous abortion） patients with a few vaginal bleeding,a small amount of decidual residue （residual maximal diam- eter〈35 mm） in the uterine cavity confirmed by B ultrasound,and β-HCG≤500.00 mlU/mL, were block randomly di- vided into two groups： 50 eases of patients in the observation group orally took drospirenone and ethinylestradiol tablets for treatment, 50 cases of patients in the control group were given mifepristone orally. Changes were observed in the two groups of vaginal bleeding and ultrasound results. Results The duration of vaginal bleeding was（4.31±1.23） days in the observation group and （8.31±1.91） days in the control group. The hemostatie rate of vaginal bleeding was 100% in the observation group and 80% in the control group. Through B-ultrasonic examination,the cure rate of intrauterine residue was 96% in the observation group and 62% in the control group. There were significant difference between the two groups in the duration of vaginal bleeding, hemostatic rate and the cure rate of intrauterine residue （P〈0.05）. Con- clusion The effect of drospirenone and ethinylestradiol tablets is vastly superior to that of mifepristone for treating in- trauterine residue after abortion. Drospirenone and ethinylestradiol tablets has an accurate effect on treating intrauterine residue,which can avoid the risk and complications of clearing uterine surgery.
