优化乳腺导管造影对良恶性乳头溢液疾病的诊断及鉴别诊断

Optimization of Mammary Gland Carcinoma Imaging of Benign and Malignant Diagnosis and Differential Diagnosis of Nipple Discharge Diseases

目的:通过优化乳腺导管造影检查获得高质量的影片资料,分析良恶性乳头溢液性疾病的造影表现特征及其诊断和鉴别诊断价值。材料与方法:回顾性分析322例非哺乳期乳头溢液的造影资料,均经临床及造影检查及病理证实。结果肿瘤性病变占26.7%(86例),以导管内乳头状瘤最多见,占肿瘤性病变的88.4%(76例);非肿瘤性疾病占55.3%(178例),以乳腺导管扩张或伴慢性导管炎最多见,占非肿瘤性疾病的78.1%(139例),生理性溢占全部乳头溢液的18%(58例)。结论优化乳腺导管造影为乳头溢液性疾病的首选检查方法,对乳导管病变的诊断具有定位定性的作用,在良恶性乳头溢液疾病的诊断和鉴别诊断中有重要价值。

Purpose： by optimizing the mammary gland catheter angiography examination to obtain high quality video data,analysis of benign and malignant imaging performance characteristics of nipple discharge diseases and its diagnosis and differential diagnosis value. Materials and Methods： a retrospective analysis of 322 cases of nursing nipple discharge imaging data,were confirmed by clinical and imaging examination,and pathology. Results： tumor lesions accounted for 26. 7%（ 86cases）,with most common intraductal papilloma,accounting for 88. 4% of the tumor lesions（ 76 cases）; Non tumor diseases accounted for 55. 3%（ 178 cases）,with mammary duct expansion or the most common chronic duct inflammation,accounted for 78. 1% of the tumor diseases（ 139 cases）,physiological overflow accounted for 18% of all nipple discharge（ 58 cases）.Conclusion： optimization of mammary gland catheter angiography is the first choice for the nipple discharge diseases,the diagnosis of mammary duct lesion with qualitative,positioning in the benign and malignancy of nipple discharge diseases has important value in diagnosis and differential diagnosis.
