摘要
目的:探讨影像学检查对鼻骨区骨折的临床应用价值。材料与方法:在80例鼻区骨折中,对70例单纯性鼻骨骨折患者行x线拍片检査。对10例鼻区复合性骨折患者行HRCT检査,层厚2mm,鼻区靶扫描,骨算法重建成像。结果:80例鼻区骨折中:单纯鼻骨骨折70例占87.5%(鼻骨橫行线样骨折60例,单纯性鼻骨粉碎性骨折10例),鼻骨粉碎性骨折合并面骨的复合性骨折10例占12.5%。其中4例上颌骨额突骨折,3例泪骨骨折和2例眼眶壁骨折,1例上颌骨鼻骨缝分离。结论:鼻骨区骨折是临床常见的损伤性病变,大多数单纯性鼻骨骨折X线平片检查可以明确诊断。鼻骨区复合性骨折常需采用CT或高分辨率CT(HRCT)检査。HRCT检査能细致地显示鼻区部骨结杓,对鼻区复合性骨折的部位丶程度及伴发改变观察全面丶准确,是具有重要临床价值的检査方法。
Purpose: To study the clinical value of Imaging examination in diagnosing fractures of nasal region. Materials and Methods: In 80 cases diagnosing injuries of the nasal region,70 cases with simple fractures of the nasal bonesshown on X-ray films were reviwed. 10 cases with complex fractures of the nasal region shown on HRCT also were revewed.HRCTperformed with slice thickness of 2mm and bone algorithm. Results: In 80 cases with the fractures of nasal region: 70 simple fractures of nasal bone,87. 5%( 60 cases of transverse fracture of the nasal bones,10 cases of comminuteted fracture of the nasal bones). In10 complex fractures with the comminuted nasal bones,12. 5% : fracture of the frontal process of maxillary bone was found in 4 cases,fracture of lacrimal bone in 3 cases and fracture of orbital walls in 2 cases. Separation of nasomaxillary suture was found in 1 case. Conclusion: The fracture of nasal bone is a common injury. The most cases of simple nasal bone coul diagnose with X-films. But complex fractures need HRCT scan. HRCT could accurately display the bony details in the nasal region including location,extent and secondary changes of complex fractures.
