乳腺导管扩张症的造影诊断及介入性药物灌注治疗

目的:分析乳腺导管扩张症造影X线影像特征特点,以提高其认识。方法:分析39例乳腺导管扩张症造影X线征象,与病理结构对照分析。对经溢液病理涂片、泌乳术测定排除肿瘤性病变,并确保无禁忌症,经临床保守治疗疗效不显著的导管扩张症病例,在造影引导下,选择溢液乳孔注入药物灌注治疗。结果:37例造影X线诊断与手病理相符,其中2例误诊为导管内乳头状瘤,诊断符合率可达94.9%。经溢液乳孔注入药物治疗,效果显著,总有效率达92.8%。结论:经选择性乳腺导管造影,可明确诊断乳腺导管扩张的程度及病变范围;对于具有适应症的导管扩张症,采用造影引导下的药物灌注治疗,疗效显著,本组研究成果具有临床推广应用价值。

Purpose： To analyze mammary duct ectasia contrast X-ray image characteristic features to enhance their understanding. Materials and Methods： 39 cases of ductal ectasia contrast X-ray findings were compared with the pathological structure. After discharge of pathological smear,lactation cytometry exclude neoplastic lesions,and to ensure that no contraindications,clinical conservative treatment effect is not significant ductal ectasia cases,under the guidance of contrast,choose milk discharge hole injection drug infusion therapy. Results： 37 cases of angiography X-ray diagnosis of pathology consistent with the hands,of which 2 cases were misdiagnosed as intraductal papilloma,diagnosis rate of 94. 9%. After discharge milk hole injection drug treatment,the effect is significant,the total effective rate was 92. 8%. Conclusion： ductal selective angiography can confirm the diagnosis of lesions and the degree of ductal expansion; having indications for dilatation catheter,use of drugs under the guidance of contrast infusion therapy,a significant effect,this group findings have clinical application value.
