摘要
目的探讨老年细菌性肺炎患者贫血发生情况及其影响因素。方法回顾性分析61例老年细菌性肺炎患者和100例老年健康体检者实验室检查结果。比较两组血红蛋白(Hgb)水平及贫血发生率。另将老年细菌性肺炎组分为贫血组和非贫血组,比较两组C反应蛋白(CRP)、清蛋白(Alb)水平及体质指数(BMI)。分析老年细菌性肺炎组Hgb与CRP、Alb、BMI及年龄的相关性。结果老年细菌性肺炎组Hgb水平低于健康对照组,贫血率高于健康对照组,差异均有统计学意义(P<0.01);老年细菌性肺炎贫血组CRP高于老年细菌性肺炎非贫血组、而Alb、BMI低于老年细菌性肺炎非贫血组,差异均有统计学意义(P<0.01);细菌性肺炎组Hgb水平与CRP呈负相关(r=-0.258,P<0.05),与Alb呈正相关(r=0.263,P<0.05),与BMI、年龄的相关性均无统计学意义(r分别为0.156、-0.039,P>0.05)。结论老年细菌性肺炎患者贫血发生率较高,营养不良、炎症反应可能是其重要原因。
Objective To investigate the incidence of anemia and its related factors in eldly patients with bacterial pneumonia.Methods A retrospective analysis of the results of laboratory tests of 61 cases of elderly patients with bacterial pneumonia and100 cases of elderly healthy individuals was carried out.The level of hemoglobin(Hgb)and the anemia rate were compared between elderly patients with bacterial pneumonia group and the control group. The elderly patients with bacterial pneumonia group was divided into anemia group and non anemia group,and Creactive protein(CRP),albumin(Alb)and body mass index(BMI)were compared between two groups.The correlation between Hgb and CRP,Alb in the elderly patients with bacterial pneumonia group.Results The level of Hgb of elderly patients with bacterial pneumonia group was significantly lower than the control group,and the anemia rate significantly higher than the control group(P0.01). The level of CRP of anemia group was significantly higher than non anemia group and the levels of Alb and BMI were significantly lower than non anemia group in the elderly patients with bacterial pneumonia(P0.01).The level of Hgb was negatively correlated with CRP(r=-0.258,P0.05),and positively correlated with Alb(r=0.263,P0.05),but the correlation between Hgb and BMI,age was not statistically significant(r=0.156,-0.039,P0.05)in elderly patients with bacterial pneumonia. Conclusion The incidence of anemia is higher in elderly patients with bacterial pneumonia,and malnutrition and inflammation is associated with anemia.
