48家医疗机构抗菌药物管理体系建设情况分析

Analysis on Establishment of Antimicrobial Stewardship in 48 Medical Institutions

目的 :了解青岛市抗菌药物管理(antimicrobial stewardship,AMS)体系建设情况。方法 :选取青岛市内48家二级以上公立医疗机构,调查抗菌药物管理工作组情况、责任状情况等13项内容,依据标准评价为达标、基本达标、不达标。结果:青岛市AMS体系建设平均达标率为64.42%,平均基本达标率为18.91%,平均不达标率为16.67%。其中三级综合医院达标率最高为93.85%,二级专科医院达标率最低为32.31%;在调查指标中,处方权和调剂权达标率最高为100.00%,抗菌药物监测网和细菌耐药监测网达标率最低为27.03%。结论:青岛市AMS体系较为完善,抗菌药物临床应用趋于合理,但各级别医院间差异大,二级医院有待加强管理。

Objective：To investigate the establishment of antimicrobial stewardship （AMS） in Qingdao city. Methods： A total of 48 public medical institutions of Classes Ⅲ and Ⅱ in Qingdao City were selected and investigated for 13 indicators including the working group of antimicrobial agents and liability form etc, of which the results were judged as qualified, basically qualified and unqualified according to the relevant standards. Results：As a whole, the mean qualification rate of establishment of AMS in Qingdao was 64.42%, while the mean basic qualification rate was 18.91% , and the mean unqualification rate was 16.67%. The highest qualification rate appeared in Class Ill comprehensive hospitals, which was 93.85%. However, the lowest qualification rate appeared in Class lI specialized hospitals, which was 32.31%. The qualification rates of authorization of prescription and dispensing were the highest in all the indicators, which were 100.00%, while those of monitoring and management of the Center for Antibacterial Surveillance and the CARSS were the lowest, which were 27.03%. Conclusion：The AMS in Qingdao City was relatively perfect, and the clinical use of antibacterial tended to be rational, which showed significant difference in the hospitals of different classes, and those in Class Ⅱ hospitals needed to be further strengthened.
