摘要
目的探讨联合检测肾损伤分子-1(KIM-1)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、肝型脂肪酸结合蛋白( LFABP)的表达对输尿管软镜术(RIRS)患者肾组织损伤的临床意义.方法筛选 2013 年 5月 -2013 年11 月,经我院治疗需要接受RIRS 的 36 例肾结石患者作为观察组,选取 40例无泌尿生殖道疾病同期健康体检者为对照组.分别检测对照组和观察组 RIRS术前、术后 2 、24 h尿中KIM-1、NGAL、NAG、LFABP和肌酸酐 (Cr)含量,并计算出KIM-1 / Cr、NGAL / Cr、NAG / Cr 和LFABP / Cr.结果RIRS术后2 h尿KIM-1 / Cr 为 5.15 ± 2.19,与对照组和术前组比较明显上升,差异有统计学意义(P〈0.05);NGAL / Cr 为 943 ± 172,与对照组和术前组比较显著上升,差异有统计学意义(P〈0.05).术后 24 小时KIM-1 / Cr 、NGAL / Cr基本恢复到术前水平,与对照组和术前组比较无显著差异(P〉0.05).且NAG / Cr和LFABP / Cr在手术后2、24 h均未显著改变(P〉0.05).结论联合检测尿 KIM-1 、NGAL、NAG和LFABP 可作为判定RIRS 患者肾损伤的敏感标志物.
Objective To Combine investigate the clinical value of the damage effects of retrograde intra-renal surgery (RIRS) on kidney tissue by measuring kidney injury molecule-1 (KIM-1), neutrophil gelatinase associated lipocalin (NGAL), N-acetyl-β-D-glucosaminidase (NAG), liver-type fatty acid binding protein (LFABP) expression. MethodsWe enrolled thirty-six consecutive patients (The observation group) who underwent RIRS that presented with renal calculi size 〈 2 cm in our hospital from may 2013 to novembre 2013. Forty control patients (The control group) with no signs or symptoms of urogenital disease were also enrolled for this study. Patients urine KIM-1, NGAL, NAG, and LFABP and creatinine levels were determined before the surgery, 2 h and 24 h after the surgery. ResultsTwo hours after the surgery KIM-1/Cr was (5.15 ± 2.19) ,NGAL/Cr was (943 ±172).The surgery KIM-1/Cr and NGAL/Cr levels had increased significantly in urine compared to levels control group and before the surgery (P〈0.05, respectively) and Basically recovered to preoperative levels 24 hours after the surgery. The NAG/Cr and LFABP/Cr levels did not change significantly after the surgery(P〉0.05). ConclusionCombined investigate of urinary KIM-1 / Cr and NGAL / Cr could be used as sensitive marker of tissue damage in patients with RIRS.
