基于Meta分析的脉络宁注射液治疗急性脑梗死临床评价研究

Meta-analysis of Efficacy of Mailuoning Injection in Treatment of Acute Cerebral Infarction

目的:综合评价脉络宁注射液治疗急性脑梗死的临床疗效及安全性。方法:计算机检索中国期刊全文数据库(CNKI)、中文科技期刊全文数据库(VIP)、万方数据库、中国生物医学文献数据库(SinoMed)、PubMed等数据库,全面检索脉络宁注射液治疗急性脑梗死的随机对照试验,采用Cochrane风险评价表评价其研究质量,提取资料并通过RevMan 5.3软件进行数据分析。结果:系统评价研究中,共纳入22篇文献,累计2 247例受试者。Meta分析结果显示:在西医常规治疗的基础上,联用脉络宁注射液对比对照组仅用西医常规,或西医常规加用其他中药注射剂,在治疗急性脑梗死方面具有较好的疗效,可有效提高临床总有效率(RR=1.15,95%CI:1.12~1.19,P<0.000 01),改善神经功能缺损情况(MD=-6.30,95%CI:-7.77^-4.83,P<0.000 01),同时,可以降低纤维原蛋白指数、胆固醇及三酰甘油等指标。结论:临床治疗急性脑梗死的过程中,在常规治疗的基础上联用脉络宁注射液可以提高疗效,但其安全性仍需进一步探讨。

Objective：To assess the clinical effictiveness and safety of Mailuoning injection combined with con- ventional medicine for treating acute cerebral infarction （ ACI ） . Methods： Datebases including CNKI, VIP, Wanfang, PubMed and so on were searched to collect randomized controlled trials （RCTs） which regarding Mailuoning injection com- bined with conventional medicine for ACI. Two reviewers independently evaluated the quality of included RCTs by The Co- chrane Risk of Bias Assessment Tool, and then extracted the required information. And Meta-analysis was performed using RevMan 5.3 software. Results： A total of 22 RCTs involved 2 247 participants were included. The results of Meta-analysis indicated Mailuoning injection combined with conventional medicine can be superior than conventional medicine alone in re- spect of total effective rate （RR = 1.15, 95% CI：1. 12-1.19, P 〈 0.000 01 ）, the degree of neurological impairment （MD = - 6.30, 95 % CI： - 7.77- - 4.83 ）, P 〈 0. 000 01 ）. Furthermore, Mailuoning injection could decrease the index of fi- brinogen, blood lipids blood fat and so on. Conclusion：In the process of the clinical treatment of acute cerebral infarction, Mailuoning injection combined with conventional medicine could improve the curative effect. But its safety still needed to be further discussed.
