危重患者访视追踪模式的构建及效果评价

Construction and effect evaluation of the follow-up model of critical patients

目的探索危重患者访视追踪模式的构建,评价其效果。方法自2012年8月起,我院采用三级访视追踪模式(即护士长-科护士长-护理部三级)对全院危重患者进行管理,2014年改进为护士长-护理部危重管理组二级访视追踪模式;评价该模式对危重患者护理质量的影响。结果通过访视追踪,2013-2014年全院每季度危重患者护理质量平均得分稳定在较好水平,科室危重患者护理质量平均得分与全院相一致;危重患者护理质量检查出现问题的频次降低。结论二级访视追踪模式在危重患者管理中提高了危重患者的护理质量,提高了护理工作效率,确保了患者安全,具有科学性和实用性。

Objective To study the construction of critical patients interviewing model and evaluated it＇s effect Methods Since August 2012, Three interview system （i. e. the head nurse, head nurses-nursing department） was used for the management of hospital critically ill patients In our hospital. In 2014, the system was modified as head nurses-nursing department critical management group two-level follow-up model. The effect of the model on the quality of nursing of critically ill patients was evaluated. Result Through the follow-up tracking, the average score of the quality of nursing care of critical patients in each quarter from 2013 to 2014 was stable in a better level, and con- sistent with the average score of the quality of care for patients with critical sections. The frequency of the problem of nursing quality examination in critical patients was decreased. Conclusion The head nurse-nurse department two level interviewing model increase the quality and efficiency of nursing, ensure patients safety. It was scientific and practical.
