2005年-2014年某医院肾内科住院患者疾病构成分析

Diseases Composition Analysis on the Hospitalized Patients in Nephrology Department of a Hospital from 2005 to 2014

目的回顾性分析某三级甲等医院肾内科近10年所有住院患者的疾病构成,为医院肾内科细化管理及未来发展规划提供科学依据。方法收集某院肾内科近10年所有住院患者病历资料,按照国际疾病分类编码(ICD-10)进行疾病分类,按年龄分为青年组、中年组、老年组,用SPSS 13.0进行统计分析。结果某院肾内科近10年共收治患者21 465例,男女性别比为1.19:1,其中老年组男女性别比为2.44:1;前7位病种收治患者合计为18 127例,占所有肾内科住院患者的84.45%。青年组狼疮性肾炎、肾病综合征、Ig A肾病构成比明显高于中老年组,而中老年组糖尿病肾病、高血压肾病、梗阻性肾病明显高于青年组,不同年龄组病种间的差异均有统计学意义。老年组死亡构成比明显高于中年组和青年组,差异有统计学意义。结论某院肾内科住院患者主要病种为肾病综合征、慢性肾炎、狼疮性肾炎、Ig A肾病、糖尿病肾病、高血压肾病、梗阻性肾病,不同年龄组性别、疾病、死亡构成存在差异。

Objectives To investigate the diseases composition of hospitalized patients in nephrology department of a Class A and Tertiary Hospital in recent ten years,and provide scientific basis for refining management and future development plan of the nephrology department. Methods The medical records materials of hospitalized patients in 〈br〉 nephrology department of a hospital in recent ten years were collected,then made diseases classification according to the ICD-10 and divided into young group,middle age group and older age group according to age,finally conducted statistical analysis with the application of SPSS 13.0. Results 21 465 patients were treated in recent ten years,proportion of male and female was 1.19：1,proportion of elderly age group was 2.44：1;18 127 patients in top seven diseases accounted for 84.45%. Proportions of lupus nephritis,nephrotic syndrome,IgA nephropathy in young were higher than middle and elderly groups; diabetic nephropathy,Proportions of hypertensive nephropathy, obstructive nephropathy in middle and elderly groups were higher than young. Conclusions The main diseases in this hospital in recent ten years were nephrotic syndrome,chronic nephritis,lupus nephritis,IgA nephropathy,diabetic nephropathy,hypertensive nephropathy and obstructive nephropathy. There were statistical differences existing in gender,diseases entities and death entities in different groups.
