摘要
目的 检测结直肠癌(colorectal cancer,CRC)癌组织中T淋巴瘤侵袭转移诱导因子1(T lymphoma invasion and metastasis inducing factor 1,Tiam1)mRNA表达量的变化,探讨其与CRC外周血微转移及预后的关系。方法 应用实时荧光定量RT-PCR检测63例CRC患者癌组织、25例结直肠良性病变组织中Tiam1 mRNA的表达量,同时应用实时荧光定量RT-PCR检测63例CRC患者外周血中角质蛋白20(CK20)mRNA表达以评价外周血微转移,CRC患者术后预后随访3年,分析CRC癌组织中Tiam1 mRNA表达量与外周血微转移及预后的关系。结果 外周血微转移阳性CRC组癌组织中Tiam1 mRNA表达量显著高于外周血微转移阴性CRC组(P〈0.01),两者均显著高于结直肠良性病变组(P〈0.01);CRC组Tiam1 mRNA表达量与肿瘤分化程度、浸润深度、有无淋巴结转移,有无远处脏器转移及有否术后复发转移呈正相关,与术后生存期长短呈负相关(P〈0.05)。单因素分析显示癌组织中Tiam1 mRNA表达量与外周血微转移相关(χ^2=11.866,P〈0.01),Cox多因素分析显示其是CRC外周血微转移独立的危险因素(Exp=5.782)。结论 癌组织中Tiam1表达量升高与CRC外周血微转移及不良预后密切相关,可成为评价CRC预后有价值的参考指标。
Objective To investigate the relationship of quantitative expression of T lymphoma invasion and metasta- sis inducing factor 1 (Tiaml) mRNA in colorectal cancer (CRC) tissues with peripheral blood micrometastasis and prognosis of CRC. Methods The quantitative expression of Tiaml mRNA in cancer tissues of 63 patients with CRC and 25 cases of colorectal benign lesion tissues was detected by real-time quantitative RT-PCR. Expression of CK20 was detected by real-time quantitative RT-PCR for evaluating the peripheral blood micrometastasis. The postoperative prognosis were followed-up for 3 years. The relation of the quantitative expression of Tiaml mRNA with the peripheral blood miro- metastasis and prognosis of CRC was investigated. Results The expression of Tiaml mRNA in the peripheral blood mi- crometastasis CRC group was significantly higher than that in the no peripheral blood micrometastasis CRC group (P 〈 0.01 ) , which was significantly higher than that in the colorectal benign lesion tissues (P 〈 0.01 ). The expression of Tiaml mRNA in the CRC group was related to the differentiated degree of tumor, depth of invasion, lymph node metastasis, distant organ metastasis, the postoperative survival time and the postoperative recurrence and metastasis (P 〈 0.05). Univariate analysis revealed that the expression of Tiaml mRNA in cancer tissues was related with peripheral blood micrometastasis of CRC (χ^2 = 11. 866,P 〈0.01 ). Multivariate Cox analysis showed that it was independent risk factors for the peripheral blood micrometastasis of CRC (χ^2 = 5. 782,P 〈 0.01 ). Conclusion High expression of Tiaml mRNA in cancer tissue was closely related to the peripheral blood micrometastasis of CRC and the poor prognosis, and it may be a valuable reference index for the prognosis of CRC.
