多西他赛注射液致过敏性休克死亡

Anaphylactic shock leading to death induced by docetaxel

1例60岁男性患者因咽癌行多西他赛＋奈达铂化疗。在首次给予多西他赛注射液（120mg加入0．9％氯化钠注射液500m1）静脉滴注5min后，患者出现呼吸困难血压测不出，旋即心跳呼吸骤停。立即停药，给予胸外按压，气管插管，呼吸机辅助呼吸，以及肾上腺素、地塞米松、多巴胺等药物治疗，抢救无效，18．5h后死亡。因患者同时未应用其他药物，故考虑过敏性休克很可能为多西他赛所致。

A 60-year-old male patient with hypopharyngeal carcinoma received chemotherapy with doeetaxel and nedaplatin. Five minutes after the first treatment with docetaxel （120 mg in 0. 9% sodium chloride 500 ml）, the patient developed dyspnea, cardiac and breath arrest, and his blood pressure was too low to be measured. The drug was stopped immediately and chest compression, endotraeheal intubation, ventilator-assisted breathing, drug therapy such as adrenaline, dexamethasone, and dopamine were given. The patient eventually died after 18.5 hours of rescue. Because the patient did not received any other drugs except doeetaxel, it was considered that anaphylactic shock was induced by docetaxel.
