β-HCG比值联合子宫内膜厚度预测早期不明位置妊娠者异位妊娠的临床价值 被引量:11

摘要 目的:探讨β-HCG比值联合子宫内膜厚度对早期不明位置妊娠者异位妊娠的预测价值。方法:从我院2011年2月至2014年12月间产生的妊娠病例中随机选择151例,回顾分析孕妇的血清β-HCG比值以及子宫内膜厚度检测方法及结果,并比较应用该种检测方法诊断宫内妊娠正常、妊娠异常以及异位妊娠孕妇的诊断符合率。结果:异位妊娠孕妇的血清β-HCG比值明显低于宫内妊娠正常孕妇,其差异具有统计学意义(P<0.05);异位妊娠孕妇子宫内膜厚度小于宫内妊娠正常孕妇,其差异具有统计学意义(P<0.05)。此外,妊娠异常孕妇的血清β-HCG比值以及子宫内膜厚度值均小于宫内妊娠正常孕妇,其差异具有统计学意义(P<0.05)。血清β-HCG比值联合子宫内膜厚度检测的诊断符合率明显高于单项血清β-HCG比值或子宫内膜厚度检测,其差异具有统计学意义(P<0.05)。此外,血清β-HCG比值以及子宫内膜厚度的诊断方法在异位妊娠检查中的诊断符合率要高于妊娠正常及妊娠异常两类,其差异具有统计学意义(P<0.05)。结论:联合β-HCG比值和子宫内膜厚度两项指标进行判断,能够在早期不明位置妊娠者异位妊娠的检测过程中发挥比较准确的预测作用,具有较高的诊断价值;同时这两项判断指标仅需联用非侵入性检测技术就能达到有效检测的目的,是妇产科检测中一种安全系数高、真实准确、值得推广的方法。 Objectives: To explore the predictive value of β- hCG ratio joint endometrial thickness for early ectopic pregnancy of unknown location. Methods: 151 pregnant women in our hospital from February 2011 to December 2014 were randomly selected, and the clinical examination and diagnosis methods and results were retrospectively analyzed, to compare the diagnostic accordance rate for normal pregnancy, abnormal pregnancy and ectopic pregnancy. Results : The serum beta HCG ratio of eetopic pregnant women was significantly lower than normal pregnant women, with statistically significant difference ( P 〈 0. 05 ). The endometrial thickness of ectopic pregnant women was significantly less than normal pregnant women, with statistically significant difference ( P 〈 0. 05 ). The beta HCG ratio and endometfial thickness of abnormal pregnant women were less than the normal pregnant women, with statistically significant difference ( P 〈 0. 05 ). The joint diagnosis accordance rate of serum beta HCG ratio and endometrial thickness was obviously higher than either alone, with statistically significant difference (P 〈 0. 05 ). In addition, the joint diagnosis accordance rate of ectopic pregnancy was higher than normal pregnancy and abnormal pregnancy, with statistically significant difference ( P 〈 0. 05 ). Conclusion : Joint use of beta hCG ratio and endometrlal thickness is of remarkable value in the prediction of early ectopic pregnancy of unknown location. Combined with noninvasive detection technology, it is an effective detection method with high safety and accuracy, which is worth promoting.
