免钛夹微小瘢痕两孔法腹腔镜胆囊切除术的临床应用

Clinical application of two-port non-titanium clip laparoscopic cholecystectomy with tiny scar

目的：探讨使用常规腹腔镜器械施行免钛夹微小瘢痕两孔法腹腔镜胆囊切除术（laparoscopic cholecystectomy,LC）的临床效果。方法：回顾分析2013年6月至2015年6月为114例患者行免钛夹微小瘢痕两孔法LC的临床资料,其中女88例,男26例;18-67岁,平均（39.3±12.7）岁。结果：112例顺利完成手术,手术成功率98.2%;2例因广泛致密粘连改为三孔法。手术时间平均（48.6±12.5）min,术中出血量平均（12.6±6.3）ml。患者术后均无并发症发生,剑突下仅留有5 mm微小瘢痕。结论：免钛夹微小瘢痕两孔法LC实用、安全,术后腹腔内不存留金属异物,切口瘢痕微小,美容效果好。

Objective： To explore the clinical effect of two-port non-titanium clip laparoscopic cholecystectomy with tiny scar using conventional laparoscopic instruments. Methods： The clinical data of 114 patients treated by two-port non-titanium clip laparoscopic cholecystectomy with tiny scar from Jun. 2013 to Jun. 2015 were retrospectively analyzed. There were 88 females and 26 males with the average age of（ 39. 3 ± 12. 7） years old（ range,18-67 years old）. Results： In the 114 patients treated by two-port non-titanium clip laparoscopic cholecystectomy with tiny scar,112 patients underwent the operation successfully with a success rate of 98. 2%,while a conversion to three-port method was required in another 2 patients because of extensive intraperitoneal adhesion. The average operation time was（ 48. 6 ± 12. 5） min and the amount of bleeding during operation was（ 12. 6 ± 6. 3） ml. All the patients recovered well after operation without any complications,only a 5 mm tiny scar was found on abdominal wall below the xiphoid process. Conclusions： Twoport non-titanium clip laparoscopic cholecystectomy with tiny scar is a useful and safe minimaly invasive operation with tiny incisional scar,good cosmetic effect and without metallic foreign bodies in abdominal cavity.
