含庆大霉素漱口液治疗结肠癌氟尿嘧啶化疗所致口腔溃疡的效果分析

Efficacy of gentamicin-containing gargle treating fluorouracil chemotherapy induced oral ulcer in colon cancer patients

目的探讨含庆大霉素漱口液治疗结肠癌氟尿嘧啶化疗所致口腔溃疡的效果。方法选择2012年8月至2016年1月在武汉市武昌医院诊治的结肠癌氟尿嘧啶化疗致口腔溃疡患者170例，根据随机数字表法分为观察组与对照组，各85例，对照组给予常规治疗，观察组在对照组治疗的基础上给予含庆大霉素漱口液治疗，3次／d，均治疗观察7d。比较2组总有效率、口腔溃疡愈合时间、不良反应发生情况和T淋巴细胞亚群水平变化。结果观察组总有效率高于对照组，差异有统计学意义[97．6％（83／85）比87．1％（74／85）]（P〈0．05）。观察组口腔溃疡愈合时间明显短于对照组，差异有统计学意义[（4．9±1．4）d比（8．1±1．9）d]（P〈0．05）。2组均未出现严重的不良反应。对照组和观察组治疗后的cDf值均高于治疗前，同时观察组高于对照组，差异均有统计学意义[（55．5±3．5）％比（47．1±5．2）％、（55．9±4．0）％比（41．4±4．6）％]（均P〈0．05）；2组治疗前后cD；值在组间与组内比较差异均无统计学意义（均P〉0．05）。结论含庆大霉素漱口液治疗结肠癌氟尿嘧啶化疗所致口腔溃疡具有很好的免疫调节作用，能促进溃疡愈合，提高总体治疗效果，同时安全性较好。

Objective To observe the clinical effect of gentamicin-containing gargle in treating fluorouracil induced oral ulcer chemotherapy in colon cancer patients. Methods Totally 170 colon cancer patients with fluorouracil chemotherapy induced oral ulcer from August 2012 to January 2016 in Wuhan Wuchang Hospital were randomly divided into observation group and control group, with 85 patients in each group. Both groups received routine treatment; the observation group had combined treatment including gentamicin-containing gargle （3 times/d）. After 7 d of treatment, total effective rate, oral ulcer healing time, T lymphocyte subsets levels and adverse reactions were analyzed. Results In observation group, total effective rate was significantly higher than that in control group[ 97. 6% （83/85） vs 87.1% （74/85）, P 〈 0. 05 ] ; oral ulcer healing time was significantly shorter than that in control group[ （4. 9 ± 1.4）d vs （8. 1 ± 1.9）d, P 〈0. 05]. After treatment, CD4＋ values were significantly higher than those before treatment in both groups [ control group： （55.5 ± 3.5 ）% vs （47. 1 ± 5.2）%, observation group： （55.9 ±4. 0）% vs （41.4 ±4. 6）% ], and the CD4＋ value in observation group was significantly higher than that in control group （ all P 〈 0. 05 ） ; CDs values had no significant difference between groups（P 〉 0. 05）. There were no severe adverse reactions in both groups. Conclusion Gentamicin-eontaining gargle can regulate immunity and promote ulcer healing; it is safe and effective in the treatment of fluorouracil chemotherapy induced oral ulcer in colon cancer patients.
