术前经口肠内营养支持对长期吸烟食管癌患者术后恢复的影响

Effect of preoperative oral enteral nutrition support on postoperative recovery of patients with long-term smoking esophageal cancer

目的：探讨术前经口肠内营养支持对长期吸烟食管癌患者术后恢复的影响。方法选取2013年4月至2015年4月共80例有长期吸烟史的食管癌患者为研究对象，随机分为两组，每组40例。观察组术前口服短肽型肠内营养剂予以营养支持2周，对照组常规日常饮食，然后行外科手术治疗，比较两组的治疗效果。结果观察组术前1 d 及术后7 d 血清白蛋白、血清前白蛋白高于对照组，差异有统计学意义（P 〈0.05）；术后7 d 淋巴细胞计数高于对照组，差异有统计学意义（P 〈0.05）；术后肺部感染的发生率及术后住院时间均低于对照组，差异有统计学意义（P 〈0.05）。结论术前经口肠内营养支持治疗长期吸烟的食管癌患者，不仅能改善患者术前的营养状况、维持良好的免疫功能，还能减少术后肺部感染、缩短住院时间，从而有利于患者术后恢复，改善预后。

Objective To investigate the effect of preoperative oral enteral nutrition support on postoperative recovery in patients with long-term smoking esophageal cancer. Methods From April 2013 to April 2015,80 esophageal cancer patients with long-term smoking history were randomly divided into two groups,with 40 cases in each group. The observation group was given preoperative oral type short peptide enteral nutrition agent for nutrition support for two weeks,and the control group took normal diet, and surgical treatment was given. The treatment effects of the two groups were compared and analyzed. Results On the first preoperative day and the seventh day after operation,the levels of serum albumin, serum prealbumin in the observation group were higher than those in the control group,and the differ-ences were significant（P 〈 0. 05）. On the seventh day after operation,the lymphocyte count in the ob-servation group was higher than that in the control group,and the difference was significant（P 〈 0. 05）. The incidence of postoperative pulmonary infection and postoperative hospital stay in the observation group were lower than those in the control group,and the differences were significant（ P 〈 0. 05）. Conclusions The preoperative enteral nutrition support for long-term smoking esophageal cancer pa-tients,it can not only improve the nutritional status of patients before surgery,maintain good immune function,but can also reduce the occurrence of postoperative pulmonary infection,shorten the length of hospital stay,so as to help patients recover and improve prognosis.
